【摘要】：目的探討在社區(qū)應(yīng)用家庭醫(yī)生管理模式對(duì)糖尿病患者進(jìn)行健康管理的效果。方法以上海市長(zhǎng)寧區(qū)天山路街道社區(qū)衛(wèi)生服務(wù)中心運(yùn)用家庭醫(yī)生分級(jí)管理的185例2型糖尿病患者作為研究對(duì)象,運(yùn)用家庭醫(yī)生管理模式對(duì)所有入選的患者進(jìn)行為期1年的糖尿病分級(jí)健康管理。對(duì)比家庭醫(yī)生分級(jí)管理前和管理后患者的健康相關(guān)行為的改變,以及包括體質(zhì)指數(shù)、血壓、空腹血糖、餐后2 h血糖、糖化血紅蛋白(Hb A1C)、血脂等在內(nèi)的健康指標(biāo)的變化。結(jié)果管理1年后患者的空腹血糖由干預(yù)前的(8.65±2.31)mmol/L降低至干預(yù)后的(6.66±1.49)mmol/L,餐后2 h血糖由干預(yù)前的(10.23±3.42)mmol/L降低至干預(yù)后的(8.72±2.26)mmol/L,Hb A1C由干預(yù)前的(7.42±1.35)%降低至干預(yù)后的(6.64±0.85)%,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論在社區(qū)通過(guò)家庭醫(yī)生管理模式對(duì)糖尿病患者進(jìn)行健康管理,能顯著地將糖尿病患者血糖控制在正常范圍內(nèi),有利于預(yù)防糖尿病并發(fā)癥的發(fā)生,提高患者的生活質(zhì)量。
[Abstract]:Objective to explore the effect of applying the family doctor management model to the health management of diabetic patients in the community. Methods 185 patients with type 2 of diabetes mellitus were classified by family doctors in the community health service center of Tianshan road street in Changning district, shanghai. Diabetic patients as subjects of study, The family doctor management model was used to manage all patients with diabetes mellitus for one year. The changes of health-related behaviors, including body mass index (BMI), blood pressure (BP), were compared before and after the management. Fasting blood glucose, 2 hours postprandial blood glucose, Results the fasting blood glucose decreased from 8.65 鹵2.31 mmol / L before intervention to 6.66 鹵1.49 mmol / L after intervention, and from 10.23 鹵3.42 mmol / L before intervention to 8.72 鹵2.26 mmol / L after intervention. A1C decreased from 7.42 鹵1.35% before intervention to 6.64 鹵0.85% after intervention, and the difference was statistically significant (P 0.05). Conclusion Health management of diabetic patients is carried out in the community through family doctor management model. It can significantly control the blood sugar of diabetic patients within the normal range, which is beneficial to prevent the occurrence of diabetic complications and improve the quality of life of the patients.
