【摘要】：目的應(yīng)用擴(kuò)散張量成像(DTI)和氫質(zhì)子磁共振波譜成像(~1H-MRS)評(píng)估肝豆?fàn)詈俗冃?HLD)患者腦灰質(zhì)核團(tuán)及白質(zhì)的微觀結(jié)構(gòu)損害。方法選取HLD患者26例及正常對(duì)照組20名行DTI及~1H-MRS成像,比較兩組不同腦區(qū)部分各向異性值(FA)、平均擴(kuò)散系數(shù)(ADC)、擴(kuò)散張量本征(E1、E2、E3)值及NAA/Cr、Cho/Cr、NAA/Cho比值的差異。結(jié)果 HLD患者雙側(cè)殼核及丘腦的FA及ADC值高于正常組,其中殼核差異有統(tǒng)計(jì)學(xué)意義(P0.05),而雙側(cè)丘腦差異無統(tǒng)計(jì)學(xué)意義(P0.05);雙側(cè)額葉白質(zhì)及胼胝體壓部FA值降低,ADC值升高,與正常組對(duì)比差異均有統(tǒng)計(jì)學(xué)意義(P0.05);HLD組各興趣區(qū)本征值均高于對(duì)照組,其中雙側(cè)殼核E1、E2、E3值、雙側(cè)丘腦E1值、雙側(cè)額葉白質(zhì)及胼胝體壓部E2、E3值差異均有統(tǒng)計(jì)學(xué)意義(P0.05);HLD組雙側(cè)殼核、雙側(cè)丘腦及胼胝體壓部的NAA/Cr比值低于正常對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論 DTI結(jié)合~1H-MRS技術(shù)可以提供HLD患者腦實(shí)質(zhì)的代謝及擴(kuò)散量化指標(biāo),更真實(shí)地反映腦實(shí)質(zhì)損害的程度。
[Abstract]:Objective to evaluate the microstructure of gray matter and white matter in patients with hepatolenticular degeneration (HLDD) by using diffusion imaging (DTI) and proton magnetic resonance spectroscopy (1H-MRS). Methods 26 patients with HLD and 20 normal controls were selected for DTI and 1H-MRS imaging. The values of partial anisotropy in different brain regions were compared between the two groups. The average diffusion coefficient was higher than that in the normal group. Results the FA and ADC values of bilateral putamen nucleus and thalamus in patients with HLD were higher than those in the normal group. There was significant difference in putamen nucleus (P 0.05), but no significant difference in bilateral thalamus (P 0.05), and the FA value of bilateral frontal white matter and corpus callosum decreased. Compared with the normal group, the difference was statistically significant (P 0.05). The intrinsic values of each region of interest in the HLD group were higher than those in the control group. There were significant differences in the bilateral putamen nucleus E _ (1) E _ (2) E _ (2) E _ (3), the bilateral thalamus E _ (1) value, the bilateral frontal white matter and the splenium callosum E _ (2 +) E _ (3) in the bilateral putamen nucleus of the bilateral putamen nucleus in the P0.05HLD group. The ratio of NAA/Cr in bilateral thalamus and corpus callosum was lower than that in normal control group (P 0.05). Conclusion DTI combined with 1H-MRS technique can provide quantitative index of cerebral parenchyma metabolism and diffusion in patients with HLD, and more truly reflect the degree of cerebral parenchyma damage.
