【摘要】：為探討護(hù)士對(duì)泌尿外科手術(shù)后患者實(shí)施病人自控鎮(zhèn)痛(patient-controlled analgesia,PCA)治療過(guò)程中提供全面護(hù)理干預(yù)的影響,本研究將我院2014年2月至2016年2月間收治的80例泌尿外科術(shù)后患者,按照手術(shù)先后順序劃分為對(duì)照組(40例)與干預(yù)組(40例),其中對(duì)照組患者接受PCA期間常規(guī)護(hù)理措施,干預(yù)組重點(diǎn)進(jìn)行全面鎮(zhèn)痛干預(yù)措施。我們通過(guò)綜合考慮患者滿意度,患者對(duì)臨床護(hù)理質(zhì)量的評(píng)價(jià)及其使用鎮(zhèn)痛泵后并發(fā)癥出現(xiàn)情況及疼痛緩解程度等情況,對(duì)PCA治療過(guò)程中護(hù)士提供全面護(hù)理干預(yù)的影響進(jìn)行了分析。研究顯示,對(duì)照組患者滿意度為77.5%,干預(yù)組為95%,干預(yù)組明顯高于對(duì)照組(p0.05);對(duì)照組患者在臨床鎮(zhèn)痛泵使用指導(dǎo)、并發(fā)癥預(yù)防、病情監(jiān)測(cè)及鎮(zhèn)痛護(hù)理等方面評(píng)分均低于干預(yù)組(p0.05);對(duì)照組并發(fā)癥發(fā)生率為15%,干預(yù)組為5%,干預(yù)組低于對(duì)照組(p0.05);且對(duì)照組患者疼痛緩解度為70%,干預(yù)組為90%,干預(yù)組緩解較好(p0.05),說(shuō)明大部分泌尿外科患者在接受了自控鎮(zhèn)痛泵的整體護(hù)理后對(duì)于服務(wù)表示滿意,且對(duì)護(hù)理質(zhì)量評(píng)價(jià)較高,并發(fā)癥發(fā)生率較低,主訴疼痛改善較好。我們的研究為改善泌尿外科患者的術(shù)后護(hù)理和治療提供了一定的臨床依據(jù)。
[Abstract]:To investigate patient-controlled analgesia for patient-controlled analgesia by nurses after urological surgery. The effect of comprehensive nursing intervention during the treatment of PCA was studied. 80 cases of urologic surgery patients were treated from February 2014 to February 2016 in our hospital. The patients in the control group were divided into control group (n = 40) and intervention group (n = 40) according to the order of operation. The patients in the control group received routine nursing measures during PCA. The intervention group focused on comprehensive analgesic intervention. We considered the patient satisfaction, the evaluation of clinical nursing quality and the occurrence of complications and pain relief after the use of analgesic pump. The effect of nurses providing comprehensive nursing intervention in the course of PCA treatment was analyzed. The results showed that the satisfaction degree of patients in the control group was 77.5 and that in the intervention group was 95%. The intervention group was significantly higher than the control group (P 0.05). The scores of clinical analgesic pump use guidance, complication prevention, condition monitoring and analgesic nursing in the control group were lower than those in the intervention group (P 0.05). The incidence of complications in the control group was 15%, in the intervention group 5%, and in the intervention group was lower than that in the control group (P 0.05). The pain relief degree of the control group was 70 and that of the intervention group was 90. The relief of pain in the intervention group was better than that in the control group (P 0.05). The results showed that the majority of urological patients were satisfied with the service after receiving the whole nursing of the patient controlled analgesia pump, and the quality of nursing was evaluated higher, and the incidence of complications was lower. Our study provides a clinical basis for the improvement of postoperative nursing and treatment of urological patients.
