超聲評(píng)估正常人群膈肌功能的研究

【摘要】:背景超聲評(píng)估膈肌功能的方法在國(guó)內(nèi)仍未廣泛開(kāi)展,我們的研究擬對(duì)正常人群通過(guò)超聲測(cè)量膈肌厚度以及膈肌移動(dòng)度來(lái)評(píng)估膈肌功能,以期得出正常范圍值,為臨床評(píng)估患者膈肌功能提供方法學(xué)參考。方法選取150位志愿者(75名男性,75名女性)為研究對(duì)象,根據(jù)肺通氣功能檢查納入受檢者,M型二維超聲測(cè)量不同呼吸時(shí)相(平靜呼吸、深吸氣)受檢者膈肌的厚度和移動(dòng)度,得出正常人膈肌厚度和移動(dòng)度的正常范圍值;分析性別、年齡、身高、體重以及BMI對(duì)膈肌厚度、移動(dòng)度的影響,并結(jié)合最大吸氣壓,提出超聲評(píng)估膈肌功能的臨床價(jià)值。結(jié)果我們收集了138位正常人(男性61人,女性77人)的完整參考數(shù)據(jù),發(fā)現(xiàn)正常人平靜呼吸時(shí)膈肌厚度為0.19±0.04cm,膈肌移動(dòng)度為1.4±0.4c m;用力吸氣后,厚度分?jǐn)?shù)為1.7 1±0.5 8,膈肌移動(dòng)度為5.1±1.2c m。結(jié)果還顯示膈肌厚度、膈肌移動(dòng)度存在顯著性別差異。我們還發(fā)現(xiàn)受檢者身高、體重、BMI影響膈肌厚度,它們與膈肌厚度呈顯著正相關(guān);而受檢者身高、體重、年齡影響膈肌移動(dòng)度,膈肌移動(dòng)度與受檢者年齡呈負(fù)相關(guān)(r=-0.243,P0.05)。此外,結(jié)果還表明膈肌厚度分?jǐn)?shù)、移動(dòng)度均與最大吸氣壓存在顯著正相關(guān)(r=0.484,r=0.489;P0.05),提示膈肌形態(tài)學(xué)參數(shù)與膈肌收縮功能有關(guān)。結(jié)論我們的研究主要提供了超聲測(cè)量的正常人膈肌形態(tài)的正常參考范圍值;性別、年齡、身高、體重會(huì)影響膈肌形態(tài)。另外,膈肌形態(tài)與膈肌收縮力有關(guān),證實(shí)超聲測(cè)量膈肌厚度、移動(dòng)度能夠評(píng)估膈肌功能,為臨床提供有價(jià)值的方法學(xué)。
[Abstract]:Background ultrasound assessment of extensive method of diaphragm function in China has not yet, our study of normal population as measured by ultrasound, the thickness of the diaphragm and diaphragmatic excursion to assess diaphragmatic function, in order to obtain the normal range, to provide a methodological reference for the clinical evaluation of patients with diaphragmatic function. Methods 150 volunteers (75 women and 75 men), as the research object, according to the pulmonary function test in subjects, measuring two-dimensional ultrasound M in different respiratory phases (quiet breathing, deep breathing) thickness and mobility of the subjects of the diaphragm, the normal range of diaphragm thickness and mobility values; gender analysis age, height, weight, and BMI on the thickness of the diaphragm, the impact of mobile degree, combined with the maximum inspiratory pressure, the clinical value of ultrasound assessment of diaphragm function proposed. Results we collected 138 normal people (61 men, 77 women) full participation Test data, found that normal quiet breathing diaphragm thickness was 0.19 + 0.04cm, diaphragmatic excursion was 1.4 + 0.4C m; after sniffing, thickness fraction was 1.71 + 0.58, diaphragmatic excursion was 5.1 + 1.2C M. results also showed that the thickness of the diaphragm, the diaphragm movement there are significant gender differences. They also found that the subjects of height, weight, BMI effect of diaphragm thickness, and the thickness of the diaphragm was positively correlated; while the subjects of height, weight, age of diaphragmatic excursion, diaphragmatic excursion and patient age was negatively correlated (r=-0.243, P0.05). In addition, it is also shown the diaphragm thickness fraction, mobility and maximal inspiratory pressure had a significant positive correlation (r=0.484, r=0.489; P0.05), suggesting that diaphragmatic morphological parameters associated with contractile function of diaphragmatic muscle. Conclusion our study mainly provides reference ultrasound measurement of the normal diaphragm shape range value; gender, age, height body. In addition, diaphragm morphology is related to diaphragm contractility. It is confirmed that diaphragm thickness and mobility can be used to evaluate diaphragm function and provide valuable clinical methods for clinic.
