【摘要】：目的本研究通過對臨床護士的訪談,深入了解她們對心電監(jiān)護儀報警的應答體驗,識別其選擇何種報警應答行為的主要影響因素,并提出有針對性的建議,為護理管理者制定有效的臨床報警管理方案提供實證依據(jù)。方法質(zhì)性研究中現(xiàn)象學研究方法。采用目的抽樣法對某市不同等級醫(yī)院的15名臨床護士進行半結(jié)構(gòu)式深入訪談,現(xiàn)場錄音,同時,注意觀察受訪者的舉止、表情變化及音調(diào)、語速方面的信息,并做好記錄。然后將錄音資料逐字逐句轉(zhuǎn)錄成文字資料,雙人核對,采用Colaizzi資料分析法對文本資料及現(xiàn)場記錄進行分析。結(jié)果經(jīng)過對訪談資料的反復閱讀、分析、反思、歸類和提煉,最終得出4個主題。1.護士對設(shè)置心電監(jiān)護儀報警的目的認識;2.護士應答心電監(jiān)護儀報警的行動線索:① 患者病情②工作職責③來自他人的壓力④自身需求⑤既往經(jīng)歷⑥患者安全3.護士應答心電監(jiān)護儀報警的障礙:①工作繁忙②不一定能及時發(fā)現(xiàn)報警③不必要的報警太多了④患方對休息的需求4.護士對不應答心電監(jiān)護儀報警的風險認識。結(jié)論護士對心電監(jiān)護儀報警存在不當應答行為,如:延遲應答、不應答、以及關(guān)閉報警等;護士對心電監(jiān)護儀報警的應答行為受到多種因素影響,包括:護士對設(shè)置心電監(jiān)護儀報警的目的認識、護士應答心電監(jiān)護儀報警的行動線索、護士應答心電監(jiān)護儀報警的障礙以及護士對不應答心電監(jiān)護儀報警的風險認識。護理管理者應該從護士的角度,理解其行為,減輕護士工作分擔,加強相關(guān)教育和培訓,提高護士對心電監(jiān)護儀報警的風險意識,提高心電監(jiān)護儀報警質(zhì)量,改善心電監(jiān)護儀報警告知體系,加強護患配合,力求實現(xiàn)報警的安全管理。
[Abstract]:Objective this study through the interview of clinical nurses, in-depth understanding of their ECG monitor alarm response experience, identify their choice of alarm response behavior of the main factors, and put forward targeted recommendations. To provide empirical evidence for nursing managers to formulate effective clinical alarm management scheme. Methods Phenomenology research method in qualitative study. 15 clinical nurses in different grade hospitals in a certain city were semi-structured by objective sampling method. In-depth interviews. At the same time, watch the interviewee's behavior, expression change and tone, speed of speech information, and make a good record. Then record the recorded data verbatim into text material, double check. Colaizzi data analysis method was used to analyze the text data and site records. Results after repeated reading, analysis, reflection, classification and refinement of the interview materials. Finally, four themes. 1. Nurses know the purpose of setting up ECG monitor alarm. 2. The action clue of nurses responding to ECG monitor alarm: 1 patient's condition 2 work responsibilities 3 pressure from others 4 self demand 5 past experience 6 patients' safety 3. The obstacle of nurses answering ECG monitor alarm: 3. 1 busy work 2 can not be found in time to alarm 3 unnecessary alarm too many patients for the rest of the demand 4. Nurses do not respond to ECG monitor alarm risk awareness. Conclusion there is a nurse alarm ECG monitor alarm. Improper response. Such as: delayed response, non-response, and turn off the alarm, etc. The response behavior of nurses to ECG monitor alarm was affected by many factors, including: nurses' understanding of the purpose of setting up ECG monitor alarm, the action clue of nurses responding to ECG monitor alarm. The obstacle of nurses responding to ECG monitor alarm and the risk awareness of nurses not responding to ECG monitor alarm. Nursing managers should understand their behavior from the perspective of nurses and reduce the sharing of nurses' work. Strengthen relevant education and training, improve nurses' awareness of the risk of ECG monitor alarm, improve ECG monitor alarm quality, improve ECG monitor alarm notification system, strengthen nurse-patient cooperation. To achieve alarm safety management.
