【摘要】：目的探討手指屈指運動聯(lián)合綜合康復(fù)訓(xùn)練對腦卒中后偏癱患者手指活動度和上肢功能的影響。方法選取2015年8月—2016年10月在信陽市中心醫(yī)院就診的110例腦卒中后偏癱患者,采用隨機數(shù)字表法將其分成聯(lián)合組和對照組,各55例。對照組僅予以綜合康復(fù)訓(xùn)練措施,聯(lián)合組在上述基礎(chǔ)上聯(lián)合手指屈指運動干預(yù)措施。比較兩組干預(yù)前后手指活動度指標[Brunnstrom腦卒中恢復(fù)分級(手功能部分)、主動活動范圍(AROM)、最大伸展角度、最大屈曲角度]及上肢功能指標[Brunnstrom腦卒中恢復(fù)分級(上肢部分)、前臂旋轉(zhuǎn)活動度、腕背伸活動度]變化情況,記錄其生活自理能力[Barthel自理能力指數(shù)(BI)]指標。結(jié)果干預(yù)后,兩組AROM、最大伸展角度、最大屈曲角度、手功能優(yōu)良率均高于干預(yù)前,且聯(lián)合組上述指標高于對照組(P0.05)。干預(yù)后,兩組前臂旋轉(zhuǎn)活動度、腕背伸活動度及上肢功能優(yōu)良率均高于干預(yù)前,且聯(lián)合組高于對照組(P0.05)。聯(lián)合組BI評分、優(yōu)良率均高于對照組(P0.05)。結(jié)論手指屈指運動聯(lián)合綜合康復(fù)訓(xùn)練能有效改善腦卒中偏癱患者手指活動度及上肢運動功能,對患者生活自理能力恢復(fù)有利。
[Abstract]:Objective to investigate the effect of finger flexion movement combined with comprehensive rehabilitation training on finger motion and upper limb function in patients with hemiplegia after stroke. 110 patients with hemiplegia after stroke. The patients were randomly divided into two groups: the combined group and the control group, 55 cases in each group. The control group was only given comprehensive rehabilitation training measures. The index of finger movement before and after intervention was compared between the two groups. [Brunnstrom stroke recovery grading (hand functional part, active range of movement, maximum extension angle, maximum flexion angle) and upper limb function index. [Brunnstrom stroke recovery grading (upper limb partial rotation, forearm rotation, wrist extension) was recorded to record the ability of self-care. [Results after intervention, the two groups had higher rate of excellent and good hand function than before intervention, including the maximum extension angle, the maximum flexion angle, the maximum flexion angle and the excellent and good rate of hand function. The above indexes in the combined group were higher than those in the control group (P 0.05). After intervention, the rotation activity of forearm, the activity of carpal dorsal extension and the excellent and good rate of upper limb function in both groups were higher than those before intervention. The BI score of the combined group was higher than that of the control group (P 0.05). Conclusion the finger flexion and finger movement combined with comprehensive rehabilitation training can effectively improve the finger movement and upper limb motor function of stroke hemiplegic patients. It is beneficial to the recovery of patients' ability to take care of themselves.
