【摘要】：目的對直腸癌高b值(200 s/mm~2)進(jìn)行初步的優(yōu)化,觀察高b值變化對直腸癌體素內(nèi)不相干運(yùn)動成像(IVIM)參數(shù)的影響。方法前瞻性研究22例直腸腺癌患者。由一名從事消化道腫瘤影像診斷專業(yè)10年的醫(yī)生,選取全部16個b值(A組:0、10、20、30、40、60、80、100、150、200、400、800、1000、1200、1500、2000)后,在b值=800的IVIM序列圖像上選取腫瘤實(shí)性成分最大的層面用于測量。保持感興趣區(qū)(ROI)不變,分別再次重新選取14個b值(B組:0、10、20、30、40、60、80、100、150、200、400、800、1000、1200)和12個b值(C組:0、10、20、30、40、60、80、100、150、200、400、800),軟件分別自動生成相應(yīng)的IVIM參數(shù)圖及參數(shù),同樣的步驟間隔一周后由同一醫(yī)生分別再重復(fù)測量一次。取每次測量的平均值用于分析。采用Bland-Altman圖對同一醫(yī)生間隔1周前后兩次測量數(shù)據(jù)的一致性及可重復(fù)性進(jìn)行分析。對A、B、C三組IVIM參數(shù)的平均值進(jìn)行單因素方差分析(ANOVA),同時進(jìn)行Bonferroni組間比較,以P0.05認(rèn)為差異有統(tǒng)計學(xué)意義。結(jié)果三組間比較,D*值95%一致性界限在A、B組和A、C組組間波動相對較大。ADC、D值95%一致性界限組間波動均不明顯。ADC、D、D*值的單因素方差分析結(jié)果 P值均0.05,差異具有統(tǒng)計學(xué)意義。f值A(chǔ)、B、C三組單因素方差分析P值0.05,差異沒有統(tǒng)計學(xué)意義。Bonferroni組間比較,D、D*、f值在A組和B組間均沒有統(tǒng)計學(xué)差異。隨著高b值數(shù)目的減少,ADC、D、D*值及標(biāo)準(zhǔn)誤增加,f值變化不明顯。結(jié)論直腸癌IVIM成像高b值變化對參數(shù)測量的可重復(fù)性影響不明顯。b值1500的選取價值有待進(jìn)一步研究。
