【摘要】:目的評(píng)價(jià)乳腺腫塊的VTI平均光密度值技術(shù)在校正乳腺影像學(xué)報(bào)告與數(shù)據(jù)系統(tǒng)(BI-RADS)分類中的臨床應(yīng)用價(jià)值。方法對(duì)經(jīng)病理證實(shí)的173例患者181個(gè)乳腺腫塊,行常規(guī)及彩色多普勒超聲檢查、進(jìn)行BI-RADS分類、VTI成像,再應(yīng)用Image J軟件測(cè)量VTI模式下腫塊的平均光密度值,并對(duì)BI-RADS分類做出調(diào)整,分析其診斷價(jià)值。結(jié)果常規(guī)BI-RADS分類、VTI平均光密度值及校正后BI-RADS分類鑒別診斷乳腺良惡性病灶的靈敏度為100%(74/74)、86.5%(64/74)、97.3%(72/74),特異度為51.4%(55/107)、77.6%(83/107)、72.9%(78/107),準(zhǔn)確度為71.3%(129/181)、81.2%(147/181)、82.9%(150/181)。結(jié)論 VTI平均光密度值技術(shù)對(duì)乳腺腫瘤的良惡性鑒別具有重要的臨床應(yīng)用價(jià)值,校正后BI-RADS分類較常規(guī)BI-RADS分類顯著提高了診斷的特異度、準(zhǔn)確度,且可減少不必要的穿刺活檢。
[Abstract]:Objective to evaluate the application of VTI mean optical density in the correction of breast imaging reporting and data system (BI-RADS). Methods 181 breast masses in 173 patients confirmed by pathology were selected. Routine and color Doppler ultrasonography were performed to evaluate the mean optical density of the mass in VTI mode by using Image J software. BI-RADS classification was adjusted to analyze its diagnostic value. Results conventional BI-RADS classification. The sensitivity of VTI average optical density and corrected BI-RADS classification in the differential diagnosis of benign and malignant breast lesions was 100 / 74 / 74 / 86. 5 / 74). 97.3 / 72 / 74, with a specificity of 51.4 / 107 / 77.6 / 83.3 / 107.70 / 72.9 / 10 / 7 / 78 / 107). The accuracy is 71.3 / 121 / 81.2 / 141 / 181). Conclusion the technique of VTI mean optical density has important clinical value in differentiating benign and malignant breast tumors. Compared with the conventional BI-RADS classification, the corrected BI-RADS classification improved the specificity and accuracy of diagnosis, and reduced the unnecessary puncture biopsy.
