【摘要】：目的系統(tǒng)分析國內外ICU患者非計劃性拔管的危險因素。方法計算機檢索Pubmed、OVID、Embase、Bio Med Central、CNKI、Wan Fang Data、VIP和CBM數(shù)據(jù)庫,收集建庫至2016年8月公開發(fā)表的關于ICU患者非計劃性拔管的相關文獻,發(fā)生非計劃性拔管者作為觀察組、未發(fā)生非計劃性拔管者作為對照組。采用Revman5.3.5軟件對相關數(shù)據(jù)進行Meta分析。結果納入16篇文獻,其中病例對照研究10篇,隊列研究6篇;觀察組1 090例,對照組4 802例。Meta分析結果顯示,觀察組男性比例、入ICU時APACHEⅡ評分≥17分的比例、接受身體約束的比例、慢性阻塞性肺疾病(慢阻肺)發(fā)生率、煩躁發(fā)生率及應用咪噠唑侖的比例均高于對照組(P均0.05),兩組年齡、高血壓發(fā)生率比較差異均無統(tǒng)計學意義(P均0.05)。結論男性、入ICU時APACHEⅡ評分≥17分、接受身體約束、伴有慢阻肺及煩躁、應用咪噠唑侖是ICU患者非計劃性拔管的危險因素。
[Abstract]:Objective to analyze the risk factors of unplanned extubation of ICU patients at home and abroad. CNKI Wan Fang data VIPs and CBM databases were collected and published in August 2016 about the unplanned extubation of ICU patients. The patients with unplanned extubation were used as observation group and those without unplanned extubation as control group. The data were analyzed by Meta with Revman5.3.5 software. The results were included in 16 articles. There were 10 case-control studies and 6 cohort studies. There were 1 090 cases in the observation group and 4 802 cases in the control group.
