【摘要】:目的:在過去10年中,自體軟骨細(xì)胞移植技術(shù)成為治療關(guān)節(jié)軟骨缺損的最佳方案;仡欁泽w軟骨細(xì)胞移植技術(shù)的發(fā)展過程并對(duì)其未來方向作以簡(jiǎn)單設(shè)想。資料來源:應(yīng)用計(jì)算機(jī)檢索Medline1957-01/2005-09的文章,檢索詞為“autologous/autogeneouschondrocyteimplantation/transplantation,car-tilagedefect,ACI”,限定文章語言種類為English;同時(shí)計(jì)算機(jī)檢索相關(guān)期刊論文1994-01/2005-09期間的相關(guān)文章,檢索詞“骨性關(guān)節(jié)炎、關(guān)節(jié)軟骨剝脫、關(guān)節(jié)軟骨缺損、關(guān)節(jié)軟骨損傷、軟骨細(xì)胞移植、自體軟骨細(xì)胞移植”,限定文章語言種類為中文。另外在google搜索引擎及有關(guān)生物醫(yī)學(xué)公司網(wǎng)站搜集相關(guān)資料。資料選擇:納入標(biāo)準(zhǔn):①軟骨細(xì)胞體外培養(yǎng)及移植的基礎(chǔ)理論。②動(dòng)物實(shí)驗(yàn)。③FDA審批文件。④臨床應(yīng)用報(bào)告。⑤患者隨訪分析。⑥自體軟骨細(xì)胞移植技術(shù)與其他治療方法療效對(duì)比。排除標(biāo)準(zhǔn):①較陳舊的文獻(xiàn)。②重復(fù)研究。資料提煉:共收集到300余篇(頁)與關(guān)節(jié)軟骨損傷、缺損及其治療相關(guān)的文獻(xiàn)及網(wǎng)頁,其中56篇(頁)符合納入標(biāo)準(zhǔn),排除的240篇(頁)文章系同一類重復(fù)性研究和近似的網(wǎng)絡(luò)文章。資料綜合:①軟骨細(xì)胞的體外大規(guī)模培養(yǎng)技術(shù)和在動(dòng)物體進(jìn)行的軟骨細(xì)胞移植實(shí)驗(yàn)為自體軟骨細(xì)胞移植在人體的進(jìn)一步研究奠定了理論基礎(chǔ)。②初期的自體軟骨細(xì)胞移植技術(shù)通過縫合自體骨膜來制造封閉關(guān)節(jié)軟骨缺損病灶的空腔,再將體外培養(yǎng)擴(kuò)增后的自體軟骨細(xì)胞混懸液注射進(jìn)入該空腔從而使細(xì)胞在病灶處形成新生軟骨。③后發(fā)展為膠原膜被用來替代自體骨膜作為自體軟骨細(xì)胞移植術(shù)中的病灶封閉材料。④近年來又研制出將體外培養(yǎng)擴(kuò)增后的自體軟骨細(xì)胞與Ⅰ/Ⅲ型膠原膜復(fù)合制成固態(tài)膜片,再將該膜用生物膠貼附在關(guān)節(jié)軟骨缺損部位,移植的細(xì)胞以膠原膜為基質(zhì)而增生,修復(fù)原軟骨缺損。⑤基質(zhì)誘導(dǎo)的自體軟骨細(xì)胞移植技術(shù)還不是最完美的方案,仍然存在不足。因此需要向更安全、有效,更易于運(yùn)輸、保存及更低制造成本等方向研究。結(jié)論:經(jīng)過10多年的不斷改進(jìn),自體軟骨細(xì)胞移植技術(shù)已經(jīng)成為目前治療關(guān)節(jié)軟骨缺損的最佳方案,其新一代的基質(zhì)誘導(dǎo)的自體軟骨細(xì)胞移植技術(shù)應(yīng)該得到臨床普及來治療這一疾病。
[Abstract]:Objective: in the past 10 years, autologous chondrocyte transplantation has been the best method for the treatment of articular cartilage defects. This paper reviews the development of autologous chondrocyte transplantation and gives a simple idea of the future direction of autologous chondrocyte transplantation. Data sources: Medline1957-01/2005-09 articles were searched by computer with the key words "autologous-autogeneouschondrocyteimplantationr transplanationr transplantationcar-tilage defectl ACI", and the Chinese Journal Full-text Database was searched by computer for articles related to the period 1994-01 / 2005-09, with the key words "osteoarthritis, articular cartilage exfoliation," Articular cartilage defect, articular cartilage injury, chondrocyte transplantation, autologous chondrocyte transplantation. In addition in the google search engine and related biomedical company website to collect relevant information. Data selection: basic theory of in vitro culture and transplantation of chondrocytes from standard 1: .2 Animal experiment .3FDA approval document .4 Clinical application report .5 patients follow up analysis of 6 autologous chondrocyte transplantation technique and its application. His treatment was compared. Exclusion Standard: 1 older document .2 duplicate study. Data extraction: more than 300 articles (pages) related to articular cartilage injury, defects and their treatment were collected, 56 of which met the inclusion criteria. The 240 excluded articles were the same type of repetitive research and similar online articles. Data synthesis of mass culture of chondrocytes in vitro and chondrocyte transplantation in animals laid a theoretical foundation for further study of autologous chondrocyte transplantation in human body. 2. Early autologous chondrocytes By suturing autologous periosteum, the technique of cell transplantation is used to seal the cavity of the articular cartilage defect. And then the suspension of autologous chondrocytes cultured in vitro was injected into the cavity, so that the cells formed new cartilage at the focus, and then developed into collagen membrane instead of autologous periosteum as autologous chondrocyte transplantation.
