系統(tǒng)化事故調(diào)查方法研究
[Abstract]:Investigation and analysis of accidents or attempted accidents play an important role in the identification of potential risks and accident control. The main purpose of the accident investigation is to understand the truth of the accident, and then to improve the deficiency of the risk control and management system, to avoid the recurrence of the same and similar accidents, rather than to focus on the attribution of responsibility or blame of a particular person. Our country still lacks the effective accident investigation method and the specialized accident investigation personnel so far, becomes the main obstacle to prevent the occupational injury and the industrial safety accident. Therefore, the main purpose of this study is to develop a set of practical and effective accident investigation methods. This paper investigates and analyzes the commonly used accident investigation methods at home and abroad, such as barrier analysis method, change analysis method, event and cause map, TRIPOD theory, systematic cause analysis technology, fault tree analysis, management negligence and risk tree. The advantages and disadvantages of event tree analysis, why tree analysis, cause tree method and accident investigation techniques are discussed. On this basis, the systematic accident investigation method is established by improving the accident and cause map and the root cause analysis by referring to the accident investigation flow suggested by I. S. Sutton. This method mainly includes six steps: preliminary investigation, investigation group, evidence collection, development time series, root cause analysis, investigation report and suggestion. In the last part of the paper, the systematic accident investigation method is used to analyze the explosion accident of the serve tube in a gas production plant, and the direct, indirect and fundamental causes of the accident are found out, and the results are satisfactory. The feasibility of the method is verified.
