【摘要】：近年來政府下大力氣開展區(qū)域衛(wèi)生信息平臺(tái)建設(shè),致力于在醫(yī)療服務(wù)機(jī)構(gòu)間進(jìn)行安全和標(biāo)準(zhǔn)化的信息共享,旨在對(duì)業(yè)務(wù)流程和醫(yī)療服務(wù)效果產(chǎn)生積極影響。但區(qū)域衛(wèi)生信息平臺(tái)的規(guī)劃設(shè)計(jì)相對(duì)傳統(tǒng),而醫(yī)療領(lǐng)域的環(huán)境和技術(shù)發(fā)展已發(fā)生巨大變化,即平臺(tái)涉及到多方利益的協(xié)調(diào)和對(duì)復(fù)雜技術(shù)組件的編排,對(duì)平臺(tái)的開發(fā)和應(yīng)用提出了挑戰(zhàn)。對(duì)此,數(shù)字化平臺(tái)和醫(yī)療信息系統(tǒng)的研究強(qiáng)調(diào)一個(gè)全面系統(tǒng)的頂層設(shè)計(jì)對(duì)促進(jìn)平臺(tái)深度應(yīng)用具有重要意義。從社會(huì)技術(shù)系統(tǒng)的視角出發(fā),以服務(wù)主導(dǎo)邏輯和網(wǎng)絡(luò)實(shí)體系統(tǒng)為理論基礎(chǔ),提出框架模型。以佛山區(qū)域衛(wèi)生信息平臺(tái)為例,對(duì)參與平臺(tái)設(shè)計(jì)、實(shí)施和應(yīng)用的相關(guān)組織和人員進(jìn)行深度訪談,通過明晰區(qū)域衛(wèi)生信息平臺(tái)中社會(huì)和技術(shù)元素的交互過程,實(shí)現(xiàn)對(duì)理論框架的深化和拓展。研究結(jié)果表明,區(qū)域衛(wèi)生信息平臺(tái)中醫(yī)療服務(wù)提供者和接受者的角色正在模糊化,平臺(tái)通過整合和分享多元化的信息、知識(shí)和制度等資源,促進(jìn)不同成員間的價(jià)值共創(chuàng)。相應(yīng)地,各項(xiàng)技術(shù)組件被編排成五層級(jí)架構(gòu),促進(jìn)平臺(tái)實(shí)現(xiàn)資源整合和再利用�；诖�,提出融合社會(huì)和技術(shù)元素的全面的區(qū)域衛(wèi)生信息平臺(tái)頂層設(shè)計(jì)架構(gòu)。將服務(wù)主導(dǎo)邏輯納入到區(qū)域衛(wèi)生信息平臺(tái)的研究,探討在數(shù)字化帶動(dòng)的醫(yī)療服務(wù)變革環(huán)境下平臺(tái)的角色價(jià)值,彌補(bǔ)了已有研究從產(chǎn)品輸出的角度分析平臺(tái)問題而導(dǎo)致的對(duì)社會(huì)屬性探討的不足。另外,強(qiáng)調(diào)平臺(tái)技術(shù)在價(jià)值共創(chuàng)研究中的關(guān)鍵作用,認(rèn)為價(jià)值共創(chuàng)要建立在整合并重新組織異質(zhì)性資源的基礎(chǔ)上,拓展了價(jià)值共創(chuàng)的研究范疇。提出的頂層設(shè)計(jì)架構(gòu)既強(qiáng)調(diào)了平臺(tái)在實(shí)踐中的服務(wù)意識(shí),也闡述了實(shí)現(xiàn)醫(yī)療服務(wù)價(jià)值共創(chuàng)的具體過程,為全國各地區(qū)的區(qū)域衛(wèi)生信息平臺(tái)建設(shè)提供參考。
[Abstract]:In recent years, the government has made great efforts to build a regional health information platform, which is dedicated to the safe and standardized information sharing among medical service institutions, aiming to have a positive impact on business processes and the effect of medical services. However, the planning and design of the regional health information platform is relatively traditional, and the environment and technological development in the medical field have changed greatly, that is, the platform involves the coordination of multiple interests and the arrangement of complex technical components. It challenges the development and application of the platform. Therefore, the research of digital platform and medical information system emphasizes that the top-level design of a comprehensive system is of great significance to promote the deep application of the platform. Based on the theory of service leading logic and network entity system, a framework model is proposed from the perspective of social technology system. Taking the Foshan regional health information platform as an example, the author conducted in-depth interviews with relevant organizations and personnel involved in the design, implementation and application of the platform, and clarified the interactive process of social and technical elements in the regional health information platform. To deepen and expand the theoretical framework. The results show that the roles of medical service providers and recipients in the regional health information platform are becoming blurred, and the platform promotes the value creation among different members by integrating and sharing diverse resources such as information, knowledge and institutions. Accordingly, technical components are organized into a five-tier architecture to facilitate resource integration and reuse on the platform. Based on this, a comprehensive top-level design framework of regional health information platform is proposed, which integrates social and technical elements. The service leading logic is integrated into the research of the regional health information platform, and the role of the platform in the digitalized medical service reform environment is discussed. It makes up for the deficiency of analyzing the platform problem from the point of view of product output. In addition, it emphasizes the key role of platform technology in the research of value creation, and points out that value creation should be established on the basis of integrating and reorganizing heterogeneous resources, thus expanding the research scope of value creation. The top-level design architecture not only emphasizes the service consciousness of the platform in practice, but also expounds the concrete process of realizing the value creation of medical service, which provides a reference for the construction of regional health information platform in various regions of the country.
