【摘要】：健康及其公平性,對人們的工作、生活意義重大。健康早已經(jīng)超出了醫(yī)療、衛(wèi)生的范疇,與社會經(jīng)濟(jì)的發(fā)展、社會及其制度的變化等有較大的聯(lián)系,越來越受到人們的重視。國際上對健康及其不平等的研究較早,其結(jié)論也表明世界各國普遍存在著健康的不平等現(xiàn)象,而我國對健康不平等的研究主要出現(xiàn)在近十年來。而收入作為人們重要的經(jīng)濟(jì)物質(zhì)基礎(chǔ),必然對衛(wèi)生健康等資源的公平分配產(chǎn)生極其重大的影響。因此,本文從收入的角度出發(fā),利用微觀的追蹤調(diào)查數(shù)據(jù),研究在我國居民收入的增加的情況下,在政策制定者對待不平等性的不同態(tài)度下,我國居民的健康狀況及不平等的程度是否有所變化,并且研究其變動情況是如何受到收入水平變化與健康水平變動的影響。 本文所做的具有一定新意的工作如下： 首先,對健康以及健康不平等的定義與相關(guān)內(nèi)涵進(jìn)行了理論綜述,介紹了國內(nèi)外多種測度居民健康水平及健康不平等程度的常用方法,進(jìn)行了相關(guān)實(shí)證的梳理。并且詳細(xì)介紹了本文所采用的研究與收入相關(guān)的健康不平等的各種指標(biāo)及方法。 其次,利用最新的中國健康和營養(yǎng)調(diào)查(CHNS)2009年的數(shù)據(jù),對城市、農(nóng)村居民的追蹤調(diào)查數(shù)據(jù)進(jìn)行實(shí)證分析,構(gòu)造QWB指數(shù)測度居民健康水平,并與2006年的數(shù)據(jù)進(jìn)行比較。分析結(jié)果表明：我國城鄉(xiāng)總體居民的平均健康水平并沒有隨著絕對收入的增加而上升,反而有所下降,但城鄉(xiāng)間的差異在減小。 第三,充分考慮不同的對待不平等的厭惡參數(shù),采用修正過的拓展集中度指數(shù)測度我國的健康不平等程度,同時(shí)測度健康績效。結(jié)果表明,我國存在親富人的健康不平等。 第四,仔細(xì)剔除CHNS2006年、2009年數(shù)據(jù)中的不連續(xù)數(shù)據(jù),利用追蹤的調(diào)查數(shù)據(jù),對2006年、2009年的集中度指數(shù)進(jìn)行動態(tài)分解。就收入分布與健康水平而言,分析不平等變動的原因,發(fā)現(xiàn)收入差距的擴(kuò)大惡化了健康的不平等。
[Abstract]:Health and its equity are of great significance to people's work and life. Health has long gone beyond the scope of medical and health care and has a greater connection with social and economic development, social and institutional changes, etc. The international study of health and its inequality is earlier, and its conclusions also indicate that health inequalities are widespread in all countries of the world, However, the research on health inequality in our country mainly appeared in the past ten years, and income, as the important economic material basis of people, must have an extremely important influence on the equitable distribution of health and other resources. From the perspective of income, this paper makes use of micro-tracing survey data to study the different attitudes of policy makers towards inequality under the increasing income of Chinese residents. Whether the health status and the degree of inequality of Chinese residents have changed or not, and how the change of income level and health level influence the change of health level. The new work of this paper is as follows:. First of all, the definition and related connotation of health and health inequality are reviewed theoretically, and various common methods to measure the health level and health inequality are introduced at home and abroad. It also introduces the indicators and methods of income-related health inequality. Secondly, using the data from the latest China Health and Nutrition Survey (CHN) in 2009, the paper makes an empirical analysis of the tracking data of urban and rural residents, and constructs a QWB index to measure the health level of residents. The results show that the average health level of urban and rural residents in China does not increase with the increase of absolute income, but decreases, but the difference between urban and rural areas is decreasing. Thirdly, considering the different aversion parameters to treat inequality, we use the modified expansion concentration index to measure the degree of health inequality and health performance in China. The results show that there is health inequality of pro-rich people in China. In 4th, the discontinuous data from the data of CHNS2006, 2009 were carefully excluded, and the concentration index of 2006 and 2009 was dynamically decomposed using the tracking data. In terms of income distribution and health level, the reasons for the change of inequality were analyzed. The widening income gap has been found to exacerbate health inequalities.
