血標本來源的廣泛耐藥革蘭陰性菌的流行病學(xué)分析

【摘要】:目的探討血標本來源耐碳青霉烯大腸埃希菌(CREC)、肺炎克雷伯菌(CRKP),廣泛耐藥銅綠假單胞菌(XDRPA)、鮑曼不動桿菌(XDRAB)的地理分布和可用抗菌藥物的最小抑菌濃度(MIC)分布。方法 2013年國內(nèi)主要地區(qū)208所綜合醫(yī)院收集來自血標本大腸埃希菌、肺炎克雷伯菌、銅綠假單胞菌、鮑曼不動桿菌臨床分離株共24113株。采用自動化儀器法檢測抗菌藥物MIC。統(tǒng)計分析4種耐藥株檢出率與地理分布、臨床科室來源、不同年齡組人群來源相關(guān)性。結(jié)果 CREC、CRKP、XDRPA和XDRAB平均檢出率分別為1.0%,5.5%,4.2%和13.7%。除CREC,余耐藥菌地理區(qū)域差異性明顯。兒童CRKP和XDRAB檢出率最高,分別為10.6%和13.1%。ICU科室各菌株檢出率均較高。55.8%CREC和22.9%CRKP亞胺培南MIC值≤4μg/m L,97.4%的XDRAB和84%的XDRPA為對亞胺培南MIC≥16μg/m L。所有CREC,CRKP和81.2%的XDRAB對替加環(huán)素敏感,其MIC90分別為0.5,2和4μg/m L。結(jié)論我國血標本來源的CRKP和XDRAB在各地均有檢出,尤以兒童組和ICU科室較明顯。替加環(huán)素對CRKP,CRE和XDRAB保持較高敏感性。
[Abstract]:Objective to investigate the geographical distribution and minimum inhibitory concentration (MIC) distribution of Acinetobacter baumannii (XDRAB), Pseudomonas aeruginosa (CRKP), widely resistant to carbapenem-resistant Escherichia coli (CREC), Klebsiella pneumoniae (Klebsiella pneumoniae) and Acinetobacter baumannii (Acinetobacter baumannii). Methods 24113 clinical isolates of Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa and Acinetobacter baumannii were collected from 208 general hospitals in China in 2013.
