【摘要】：目的分析2012-2014年我院臨床用血情況,探討臨床科室用血規(guī)律、特點(diǎn)及存在的問題,使醫(yī)院更好地對血液資源進(jìn)行有效管理與調(diào)配,保障科學(xué)、安全、合理用血,提高輸血治療技術(shù),節(jié)約血液資源。方法采用回顧性調(diào)查方式,收集2012-2014年我院20008例住院患者異體用血資料,使用SPSS19.00軟件進(jìn)行統(tǒng)計學(xué)分析。結(jié)果①3年來用血總量隨住院人數(shù)的增多逐年溫和上升,成分血以紅細(xì)胞比例最大(44.10%),其余依次為血漿(34.99%)、冷沉淀(14.88%)、血小板(6.03%)。②3年來女性用血總量、用血人數(shù)及輸血患者人均用血量均小于男性,性別差異有統(tǒng)計學(xué)意義。③輸血病例數(shù)隨年齡增加而逐漸增大,60歲及以上的用血人數(shù)比例最大(42.23%-45.18%),年齡差異有統(tǒng)計學(xué)意義。④內(nèi)科用血總量、輸血患者百分率、輸血患者人均用血量及住院患者人均用血量均高于外科。血液科、感染科、重癥醫(yī)學(xué)科/急診科、胸心外科、消化內(nèi)科為我院用血總量最大的幾個科室,用血總量達(dá)全院的51.29%-57.40%。全院用血量最大的是血液科,3年來分別占全院用血總量的14.37%、16.45%、17.05%,其中紅細(xì)胞、血小板的用血量均居全院第一。各臨床科室主要應(yīng)用的成分血不同：血液科、消化內(nèi)科以紅細(xì)胞為主,分別占各科室用血總量的50.06%-58.90%以及51.36%-55.11%；重癥醫(yī)學(xué)科/急診科、感染科、胸心外科以血漿為主,分別占各科室用血總量的50.29%-55.48%、63.26%-82.03%、53.86%-57.39%。⑤從用血總量上看,用血人數(shù)以3-9個單位組所占比例最大,其中,1u組、3-9u組所占比例逐年下降,2u組及9u組所占比例逐年升高。結(jié)論 ①我院3年來用血總量呈溫和上升趨勢,每年以4.04%-5.72%的速度遞增,總體朝著科學(xué)、合理的方向發(fā)展。醫(yī)院及輸血科應(yīng)根據(jù)既往用血情況制定用血計劃,滿足臨床用血治療需求。②醫(yī)院及輸血科應(yīng)根據(jù)臨床用血規(guī)律及特點(diǎn),對不同科室實行合理、安全用血的重點(diǎn)培訓(xùn)及監(jiān)管,制定進(jìn)一步可行的臨床用血管理方案。
[Abstract]:Objective to analyze the clinical use of blood in our hospital from 2012 to 2014, to discuss the rules, characteristics and existing problems of blood use in clinical departments, so as to ensure the effective management and allocation of blood resources in hospitals so as to ensure the scientific, safe and reasonable use of blood. Improve blood transfusion therapy technology and save blood resources. Methods the data of 20008 inpatients with allogeneic blood from 2012 to 2014 were collected and analyzed by SPSS19.00 software. Results the total amount of blood used in the past 13 years increased mildly with the increase of the number of inpatients. The proportion of red blood cells was the largest (44.10%), the rest was plasma (34.99%), cold precipitation (14.88%). Platelet (6.03%). In the last 23 years, the total amount of blood used in women, the number of blood users and the amount of blood used in patients with blood transfusion were lower than those in men, and the gender difference was statistically significant. 3 the number of cases of blood transfusion gradually increased with the increase of age. The proportion of patients aged 60 and above was the largest (42.23-45.18%), and the age difference was statistically significant. 4 the total amount of blood used in internal medicine, the percentage of patients with blood transfusion, The per capita blood consumption of blood transfusion patients and inpatients were higher than that of surgery. Hematology department, infection department, intensive medicine department / emergency department, thoracic and cardiac surgery, and digestive department were the departments with the largest amount of blood in our hospital, and the total amount of blood used was 51.29- 57.40% of the total amount of blood used in the whole hospital. The blood volume of the whole hospital was the largest, which accounted for 14.37% of the total blood volume of the whole hospital in the past 3 years, and 16.45% 17.05% of the total blood volume, among which the red blood cell and platelet volume were the first in the whole hospital. The main components of blood used in clinical departments were different: in hematology department, erythrocyte was the main component in digestive department, which accounted for 50.06- 58.90% and 51.36-55.11% of the total blood used in each department respectively; The departments of intensive medicine / emergency, infection, chest and heart surgery were dominated by plasma, accounting for 50.29-55.48% of the total amount of blood used in each department, respectively, and 63.26-82.03and 53.86-57.39.5% of the total amount of blood used. The proportion of blood in 3-9 units group was the largest, among which, the proportion of 1u group, 3-9u group decreased year by year, the proportion of 2u group and 9u group increased year by year. Conclusion (1) the total amount of blood used in our hospital has a mild upward trend in the past three years, increasing at the rate of 4.04-5.72% per year, and developing towards the direction of science and rationality. Hospitals and blood transfusion departments should draw up blood use plans according to past blood use conditions to meet the needs of clinical blood therapy. 2 hospitals and blood transfusion departments should, according to the rules and characteristics of clinical blood use, carry out key training and supervision on reasonable and safe use of blood in different departments. To establish a further feasible clinical blood management program.
