護理群體亞健康相關影響因素研究

【摘要】:目的:調查省級三級甲等綜合醫(yī)院護理群體亞健康狀況,重點是對其相關影響因素進行分析,為開展護理群體亞健康干預性研究提供基礎,并為制定和實施護理群體亞健康防治措施提供目標和依據(jù)。方法:選用SF-36量表、護士職業(yè)倦怠量表和護士工作應激源量表,采用典型抽樣方法,組建調查團隊,選擇五所省級三級甲等綜合醫(yī)院,共調查護士568名。在調查前、調查過程、資料整理、論文書寫四個階段實施質量控制,確保研究的真實、準確、合理。在數(shù)據(jù)統(tǒng)計與分析中,采取SPSS 17.0版統(tǒng)計學軟件對研究所獲取的數(shù)據(jù)進行統(tǒng)計學分析。采用Pearson方法進行相關分析。采用多元線性回歸分析,確定護理群體亞健康的相關影響因素。結果:1.在單因素分析中,將年齡、工齡、科室、工作單位、學歷、職稱、聘用形式、個人收入八個人口學特征分別進行分組,對每個人口學特征采取方差分析后,采取LSD法兩兩比較。P0.05為差異,P0.01為顯著性差異,二者均有統(tǒng)計學意義。2.在相關性研究中,通過Pearson相關分析,發(fā)現(xiàn)SF-36量表的八個維度與職業(yè)倦怠量表的七個維度均成負相關(P0.01);SF-36量表中的生理機能與工作性質成正相關(P0.05);生理職能與工作性質以及工作負荷成正相關(P0.05);社會功能與人際關系、護理期望成正相關(P0.05)。SF-36量表八個維度與工作應激源感受中的六個維度均成負相關(P0.01);SF-36量表中的健康變化指標與護士工作應激源影響度中的六個維度均成正相關(P0.01)。3.在多因素分析中,首先以健康變化指標為因變量,以各量表的維度為自變量進行多元線性回歸分析,分析出8個影響維度;然后分別以SF-36量表健康變化指標和八個維度為因變量,以各量表中的條目為自變量進行多元線性回歸分析,分析出健康變化指標有11個影響因素;生理機能有13個影響因素;生理職能有10個影響因素;軀體疼痛有8個影響因素;一般健康狀況有16個影響因素;精力有14個影響因素;社會功能有18個影響因素;情感職能有7個影響因素;精神健康有15個影響因素。結論:1.26~30歲年齡組、5~10年工齡組、護師組和主管護師組的亞健康狀況是最為嚴重的;在典型抽樣的六個科室中兒科和急診科兩個科護士的亞健康狀也是最嚴重;另外,個人收入、家庭收入、醫(yī)院類別也影響著護理群體亞健康;緵]有差異的是聘用形式、性別、民族、職務、住房面積、戶口所在地。2.護士的職業(yè)倦怠程度與亞健康程度呈正相關,即是說,職業(yè)倦怠程度越高,亞健康越嚴重。應激源影響度與亞健康程度呈正相關,也就是,應激源影響度越大,亞健康越嚴重。3.護理群體亞健康相關影響因素重點體現(xiàn)在“護士感到身體不適”、“工作讓護士變得對待家人性情暴躁”、“護士變得神經質,總是處于爆發(fā)的邊緣”、“護理工作范圍廣”這四個條目上。這些條目所屬的維度分別是軀體、家庭、精神、工作性質四個方面。
[Abstract]:Objective: to investigate the sub-health status of nursing population in provincial Grade 3A general hospital, and to analyze the related influencing factors so as to provide the basis for the research on sub-health intervention of nursing group. It also provides the goal and basis for the establishment and implementation of measures for the prevention and treatment of sub-health in nursing groups. Methods: SF-36 scale, Nurses Burnout scale and Nurses' Job stressor scale were selected. The investigation team was set up by using typical sampling method, and 5 provincial general hospitals of Grade 3A were selected. A total of 568 nurses were investigated. Before the investigation, the investigation process, the data arrangement, the paper writing carries out the quality control in four stages, guarantees the research to be true, accurate, reasonable. In the process of data statistics and analysis, the statistical software of SPSS 17.0 was used to analyze the data obtained by the Institute. Correlation analysis was carried out by Pearson method. Multivariate linear regression analysis was used to determine the related factors of subhealth in nursing population. The result is 1: 1. In univariate analysis, eight demographic characteristics of age, length of service, department, work unit, educational background, professional title, employment form and personal income were grouped, and each demographic characteristic was analyzed by variance analysis. Adopt LSD method to compare pairwise. P0.05 is the difference, P0.01 is the significant difference, both have statistical significance. 2. In the correlation study, Pearson correlation analysis showed that the eight dimensions of SF-36 scale were negatively correlated with the seven dimensions of job burnout scale (P0.01), the physiological function of SF-36 scale was positively correlated with job nature (P0.05). Physiological function was positively correlated with work nature and workload (P0.05); There was a positive correlation between social function and interpersonal relationship and nursing expectation (P0.05). The eight dimensions of SF-36 scale were negatively correlated with six dimensions of job stressor perception (P0.01). There was a positive correlation between the health changes in SF-36 scale and the six dimensions of the influence degree of the nurses' work stressor (P0.01). In the multivariate analysis, the health change index is taken as the dependent variable, and the dimension of each scale is taken as the independent variable to carry out multivariate linear regression analysis, and eight influencing dimensions are analyzed. Then the health change index and eight dimensions of SF-36 scale were taken as dependent variables, and the items in each scale were taken as independent variables for multivariate linear regression analysis, and 11 influencing factors were analyzed. There were 13 influencing factors in physiological function, 10 in physiological function, 8 in somatic pain, 16 in general health, 14 in energy and 18 in social function. There are 7 influencing factors in affective function and 15 in mental health. Conclusion: 1.The subhealth status of 30 years old group, 5 ~ 10 years old group, nurse group and director nurse group is the most serious, and the subhealth status of nurses in pediatric department and emergency department is the most serious in the six departments of typical sampling. In addition, personal income, family income, hospital categories also affect subhealth care groups. There is basically no difference in the form of employment, gender, nationality, position, housing area, residence location. 2. The degree of job burnout of nurses is positively correlated with the degree of sub-health, that is, the higher the degree of job burnout, the more serious sub-health. The influence degree of stress source is positively correlated with the degree of sub-health, that is, the greater the influence of stress source, the more serious sub-health. The influencing factors of sub-health in nursing group are mainly reflected in "nurses feel unwell", "work makes nurses become irritable towards their families" and "nurses become neurotic and always on the verge of outbreak". There are four items on the "wide range of nursing work". These items belong to four dimensions: body, family, spirit and nature of work.
