冰凍解凍去甘油紅細(xì)胞使用MAP懸浮液對(duì)紅細(xì)胞保存期的影響

【摘要】:目的評(píng)價(jià)MAP(枸櫞酸三鈉-枸櫞酸-葡萄糖-磷酸二氫鈉-氯化鈉-腺嘌呤-甘露醇)作為冰凍解凍去甘油紅細(xì)胞懸浮保養(yǎng)液對(duì)血液生化指標(biāo)的影響,參照《全血及成分血質(zhì)量要求》,探討使用MAP作為懸浮液的冰凍解凍去甘油紅細(xì)胞保存期。方法實(shí)驗(yàn)用60袋血液均來(lái)自蘇州市無(wú)償獻(xiàn)血人群,由400ml全血制備的2u(單位)懸浮紅細(xì)胞。于采血后6天內(nèi),2-6℃保存,制備冰凍紅細(xì)胞。解凍后的紅細(xì)胞平均分為2袋,分別用MAP和NS(0.9%氯化鈉注射液)懸浮保存,故本研究分為MAP組(n=60)和NS組(n=60)。M AP組以MAP和紅細(xì)胞按1:2的比例加入MAP液懸浮保存,放置在4℃冰箱儲(chǔ)存,分別于0d、1d、7d、14d、21d、28d、35d從每袋血液無(wú)菌操作取樣,測(cè)定并觀察Hb、F Hb、HCT、K+、Na+、溶血率質(zhì)量指標(biāo)的變化。NS組為對(duì)照組分別于0d、1d、7d無(wú)菌操作取樣,測(cè)定并觀察Hb、FHb、HCT、K+、Na+、溶血率質(zhì)量指標(biāo)的變化。結(jié)果隨著保存時(shí)間的延長(zhǎng),MAP組實(shí)驗(yàn)數(shù)據(jù)發(fā)現(xiàn)FHb在1d、7d時(shí)遠(yuǎn)遠(yuǎn)低于NS組的數(shù)值,可以說(shuō)明MAP組在1d、7d時(shí)被破壞的紅細(xì)胞數(shù)量少于NS組,并且在1d時(shí)NS組就有3/60樣本就超過(guò)了成分血質(zhì)量標(biāo)準(zhǔn)的數(shù)值。Hb在1d、7d時(shí)MAP組均高于NS組的數(shù)值,HCT在1d、7d時(shí)MAP組略高于對(duì)照組的數(shù)值,溶血率在1d、7d時(shí)MAP組低于NS組的數(shù)值,均可以證明MAP組在1d、7d時(shí)紅細(xì)胞總數(shù)量高于NS組。K+在1d、7d時(shí)實(shí)驗(yàn)組低于對(duì)照組的數(shù)值,也可以說(shuō)明MAP組的紅細(xì)胞破損、老化的較NS組少。甘油殘留量均符合要求,無(wú)菌實(shí)驗(yàn)為陰性。結(jié)論MAP液用于懸浮保存冰凍解凍紅細(xì)胞,有較好的保護(hù)作用,優(yōu)于目前使用的NS液。根據(jù)《全血及成分血質(zhì)量要求》,使用MAP液懸浮保存冰凍解凍紅細(xì)胞,各項(xiàng)質(zhì)量指標(biāo)的實(shí)驗(yàn)數(shù)據(jù)在7d內(nèi)均符合要求。
