【摘要】：目的:通過評價和比較傳統(tǒng)手部肌力訓練方法和計算機輔助訓練方法對手功能恢復(fù)的作用,探討計算機輔助肌力訓練對斷腕斷掌再植患者手功能的影響。方法:選擇24例斷腕斷掌再植患者,隨機分為治療組和對照組,均接受常規(guī)物理因子、關(guān)節(jié)活動度、日常生活相關(guān)性治療性活動等康復(fù)治療。治療組采用自主研制的計算機輔助裝置進行握力、捏力訓練,而對照組采用傳統(tǒng)的方法(使用彈力治療帶、治療泥等)進行訓練,兩組均為每次20min,一周5次,共4周。治療前及治療4周后進行評定,包括關(guān)節(jié)總主動活動度(TAM)、側(cè)捏力、握力、日常生活活動(ADL)能力以及上肢功能指數(shù)(UEFI)評定。結(jié)果:組內(nèi)比較,兩組患者治療4周后TAM、側(cè)捏力、握力、ADL評分和UEFI評分均較治療前改善(P0.05)。與對照組相比,治療組側(cè)捏力、握力、ADL、UEFI評分提高幅度更大,差異有顯著意義(P0.05)。而兩組的TAM改善程度無顯著差異(P0.05)。結(jié)論:計算輔助肌力訓練能更好地增強斷腕斷掌再植患者的手部力量,更有效地提高手功能,是值得推廣的一種治療方法。
[Abstract]:Objective: to evaluate and compare the effects of traditional hand muscle strength training method and computer assisted training method on hand function in patients with replantation of broken wrist and metacarpal. Methods: Twenty-four patients with replantation were randomly divided into treatment group and control group. All patients were treated with routine physical factors, joint activity, daily life related therapeutic activities and so on. In the treatment group, the grip strength and kneading force were trained with a computer-aided device developed by ourselves, while the control group was trained with traditional methods (using elastic treatment belt, treatment mud, etc.). The two groups were treated for 20 mins, 5 times a week for 4 weeks. The total active motion of the joint was evaluated before and 4 weeks after treatment, including the total active range of joint (TAM), side pinch strength, grip strength, daily life (ADL) ability and upper limb function index (UEFI). Results: after 4 weeks of treatment, TAM, side pinch strength, grip strength, ADL score and UEFI score were improved in both groups (P0.05). Compared with the control group, the side pinch strength, grip strength and ADLL UEFI score in the treatment group were significantly increased (P0.05). There was no significant difference in the improvement of TAM between the two groups (P0.05). Conclusion: Computer-assisted muscle strength training can enhance the strength of the hand and improve the hand function of the patients with replantation of broken wrist and metacarpal more effectively. It is a treatment method worth popularizing.
