【摘要】：目的探討不同濃度氯己定口腔護理液在經(jīng)口氣管插管患者中的應用效果。方法2012年1月—2013年12月某院綜合ICU收治的經(jīng)口氣管插管行機械通氣時間48 h以上患者,共納入120例符合條件患者,隨機分為3組,每組各40例。使用不同濃度氯己定口腔護理液應用于各組患者:試驗組(2%)、對照組Ⅰ(0.2%)、對照組Ⅱ(0.12%)。觀察并比較3組患者在口臭、口腔黏膜感染、呼吸機相關肺炎(VAP)發(fā)生時間及VAP發(fā)病率的差異。結果試驗組與對照組Ⅰ、對照組Ⅱ患者的VAP、早發(fā)VAP的發(fā)病率比較差異有統(tǒng)計學意義(均P0.05);進一步兩兩比較,對照組Ⅱ患者VAP發(fā)病率高于試驗組(47.50%VS 20.00%,P=0.009)。結論應用2%氯己定與有效的沖洗加擦拭口腔護理方法可有效降低經(jīng)口氣管插管患者的VAP發(fā)病率。
