簡易促通裝置對痙攣型偏癱患兒手功能康復(fù)的作用

【摘要】:目的:研制簡易手功能訓(xùn)練裝置,并探討此裝置對痙攣型偏癱患兒手功能康復(fù)的效果。方法:選取4—16歲痙攣型偏癱患兒17例,按隨機數(shù)表法分為實驗組(9例)和對照組(8例)。試驗組進行簡易促通裝置和常規(guī)康復(fù)訓(xùn)練,各1次/天;對照組進行常規(guī)康復(fù)訓(xùn)練2次/天。治療前和治療8周后,采用簡易上肢功能評價(STEF)、改良Ashworth分級(MAS)、殘疾兒童綜合功能評定(ADL)及表面肌電圖(s EMG)分別對兩組患兒患手的運動功能、大魚際肌群的肌張力、日常生活活動能力和大魚際肌群的耐力進行評定。結(jié)果:治療后,試驗組STEF評分、ADL評分、MAS分級和患手大魚際肌群表面肌電的均方根值(RMS)、平均頻率(MEF)與治療前比較,差異均具有顯著性意義(P0.05);且兩組間治療后MEF比較,及治療前、后MEF差值比較,差異具有顯著性意義(P0.05)。結(jié)論:簡易促通裝置可降低痙攣型偏癱患兒患手大魚際肌群的肌張力,提高肌肉的耐力,提高運動功能和ADL能力。
[Abstract]:Objective: to develop a simple hand function training device and to explore the effect of hand function rehabilitation in children with spastic hemiplegia. Methods: 17 children aged 4-16 with spastic hemiplegia were randomly divided into experimental group (n = 9) and control group (n = 8). The experimental group was given simple facilitation device and routine rehabilitation training once a day, and the control group was treated with routine rehabilitation training twice a day. Before and after 8 weeks of treatment, simple upper limb function evaluation was used to evaluate the comprehensive function of (ADL) and surface electromyography (s EMG) in children with disabilities by (STEF), modified Ashworth grade (MAS),. The motor function of the affected hands and muscle tension of thenar muscles were evaluated by (ADL) and surface electromyography (s EMG), respectively. Activities of daily life and endurance of thenar muscles were evaluated. Results: after treatment, there were significant differences in STEF score and (RMS), mean root mean square (RMS) (MEF) between the two groups after treatment (P0.05), and between the two groups before and after the treatment, and before and after the treatment, there were significant differences between the two groups in the mean root mean square (RMS) value of the surface myoelectric activity of the hypothenar muscle group (P05), and before and after the treatment, there were significant differences between the two groups. The difference of post MEF difference was significant (P0.05). Conclusion: the simple facilitation device can reduce the muscle tension of the thenar muscles of the children with spastic hemiplegia, improve the endurance of the muscles, improve the motor function and the ability of ADL.
