20株耐藥陰溝腸桿菌可移動耐藥元件檢測及樣本聚類分析

【摘要】:目的調(diào)查一組耐藥陰溝腸桿菌中可移動耐藥元件(可移動遺傳元件遺傳標(biāo)記與耐藥基因)的攜帶情況,以及本組菌株的親緣關(guān)系。方法收集醫(yī)院2015年1至12月住院病人痰液標(biāo)本中分離的耐藥陰溝腸桿菌共20株,采用聚合酶鏈反應(yīng)(PCR)及序列分析的方法分析11種可移動遺傳元件與20種β-內(nèi)酰胺類耐藥基因、16種氨基糖苷類耐藥基因。并對結(jié)果進(jìn)行了樣本聚類分析。結(jié)果有19株陰溝腸桿菌檢出可移動遺傳元件遺傳標(biāo)記、β-內(nèi)酰胺類和氨基糖苷類耐藥基因。20株耐藥陰溝腸桿菌共檢出5種可移動遺傳元件遺傳標(biāo)記:tnp513、tnpU、IS26、IS903、intⅠ1,4種β-內(nèi)酰胺類耐藥基因:blaTEM、blaOXA-1群、blaDHA、blaACT/MIR群,6種氨基糖苷類耐藥基因:aac(3)-Ⅱ、aac(6′)-Ⅰb、ant(3″)-Ⅰ、ant(2″)-Ⅰ、aph3′-Ⅵ、rmtB。樣本聚類分析提示,除2號菌與18號菌外,其余18株顯聚集性。3-9-10-16號菌株,1-17號菌株,5-14號菌株,15-19號菌株均為克隆傳播。結(jié)論本組耐藥陰溝腸桿菌檢出的可移動耐藥元件是導(dǎo)致對β-內(nèi)酰胺類、氨基糖苷類耐藥的重要原因,并且耐藥表型和基因型一一對應(yīng)。本組菌中4個不同的克隆傳播,有醫(yī)院感染的可能。
