透明敷料不同更換頻率對(duì)新生兒中心靜脈導(dǎo)管相關(guān)性感染的影響

【摘要】:目的探討新生兒經(jīng)外周靜脈置入中心靜脈導(dǎo)管(PICC)后,透明敷料不同更換頻率對(duì)導(dǎo)管感染發(fā)生的影響,為透明敷料選取合適的更換頻率提供理論依據(jù)。方法選取2012年6月-2015年6月在醫(yī)院接受留置PICC導(dǎo)管的450例新生兒為研究對(duì)象,按照隨機(jī)的方式將其平均分成3組,其中Ⅰ組每3d更換一次敷料,Ⅱ組每5d更換一次敷料,Ⅲ組每7d更換一次敷料,分析比較3組新生兒導(dǎo)管相關(guān)感染的發(fā)生。結(jié)果 450例新生兒中共有21例發(fā)生導(dǎo)管口感染,感染率為4.67%;3組新生兒病原菌檢出均以表皮葡萄球菌最多,分別占57.14%、71.42%、49.99%,其次為肺炎克雷伯菌,分別占42.86%、14.29%、16.67%;發(fā)生導(dǎo)管相關(guān)的血液感染,Ⅰ、Ⅱ組感染率均為0.67%,Ⅲ組未發(fā)生導(dǎo)管相關(guān)的血流感染。結(jié)論建議新生兒中心靜脈導(dǎo)管的敷料更換頻率為每7天1次,減少醫(yī)護(hù)人員的工作量,降低醫(yī)院感染率,敷料出現(xiàn)潮濕、松動(dòng)等現(xiàn)象時(shí),應(yīng)及時(shí)更換。
[Abstract]:Objective To investigate the effect of different frequency of transparent dressing on catheter infection after peripherally inserted central catheter (PICC) in neonates, and to provide theoretical basis for selecting appropriate frequency of transparent dressing. The incidence of catheter-related infection was analyzed and compared among the three groups. Results 21 cases of catheter-related infection occurred in 450 neonates, the infection rate was 4.67%. Staphylococcus epidermidis accounted for 57.14%, 71.42%, 49.99% respectively, followed by Klebsiella pneumoniae, 42.86%, 14.29%, 16.67% respectively; catheter-related blood infections occurred in group I and II, the infection rate was 0.67%, and no catheter-related bloodstream infections occurred in group III. Conclusion It is suggested that the dressing frequency of central venous catheter should be changed once every 7 days. It is necessary to change the dressing in time when the workload of medical staff is reduced, the nosocomial infection rate is reduced, and the dressing is wet and loose.
