【摘要】：目的探討泛耐藥銅綠假單胞菌β-內(nèi)酰胺類與氨基糖苷類耐藥基因與可移動遺傳元件的攜帶及菌株間的親緣關系,為臨床治療提供參考依據(jù)。方法收集2014年1-12月連云港市第二人民醫(yī)院患者痰液標本分離出的22株泛耐藥銅綠假單胞菌,采用聚合酶鏈反應(PCR)及序列分析的方法分析12種β-內(nèi)酰胺類耐藥基因、6種氨基糖苷類耐藥基因、3種可移動遺傳元件遺傳標記,并對檢測結(jié)果作樣本聚類分析,數(shù)據(jù)采用SPSS 17.0軟件進行統(tǒng)計分析。結(jié)果 22株泛耐藥銅綠假單胞菌共檢出6種β-內(nèi)酰胺類耐藥基因、5種氨基糖苷類耐藥基因、3種可移動遺傳元件遺傳標記,且陽性率較高;樣本聚類分析提示22株泛耐藥銅綠假單胞菌分為A1、A2、B的3個家族,這3個家族均提示為醫(yī)院感染。結(jié)論泛耐藥銅綠假單胞菌中攜帶的耐藥基因和耐藥表型相對應,攜帶blaIMP、blaTEM、blaCARB、blaOXA-10群、blaVEB基因和伴膜孔蛋白基因oprD2缺失以及攜帶若干個氨基糖苷類耐藥基因,是泛耐藥銅綠假單胞菌對β-內(nèi)酰胺類和氨基糖苷類耐藥的重要原因。
[Abstract]:Objective to investigate the relationship between 尾 -lactam and aminoglycoside resistant genes and transportable genetic elements in pan-resistant Pseudomonas aeruginosa and to provide reference for clinical treatment. Methods A total of 22 pan-resistant Pseudomonas aeruginosa strains isolated from sputum of patients in Lianyungang second people's Hospital from January to December 2014 were collected. Polymerase chain reaction (PCR) and sequence analysis were used to analyze 12 尾 -lactam resistance genes, 6 aminoglycoside resistance genes and 3 transportable genetic element genetic markers. The data were analyzed by SPSS 17.0 software. Results A total of 6 尾 -lactam resistant genes, 5 aminoglycoside resistance genes and 3 transportable genetic elements were identified in 22 pan-resistant Pseudomonas aeruginosa strains, and the positive rate was high. Cluster analysis showed that 22 strains of pan-resistant Pseudomonas aeruginosa were classified into three families of A1A2B, all of which were identified as nosocomial infection. Conclusion the drug-resistant genes contained in pan-resistant Pseudomonas aeruginosa correspond to the phenotypes of drug resistance. They carry the oprD2 deletion of blaVEB gene and membrane porin gene and several aminoglycoside resistance genes. Pan-resistant Pseudomonas aeruginosa is an important cause of 尾-lactam and aminoglycoside resistance.
