甲狀腺術(shù)后切口耐甲氧西林金黃色葡萄球菌耐藥基因研究

【摘要】：目的 了解甲狀腺術(shù)后切口感染耐甲氧西林金黃色葡萄球菌(MRSA)耐藥及耐藥基因檢測(cè)情況。方法 收集2011-2014年長(zhǎng)春地區(qū)醫(yī)院甲狀腺術(shù)后切口感染標(biāo)本,采用西門子公司MicroScan WalkAway-40全自動(dòng)微生物鑒定系統(tǒng)鑒定細(xì)菌,采用K-B法對(duì)MRSA進(jìn)行藥敏試驗(yàn);提取細(xì)菌DNA,設(shè)計(jì)特異性基因進(jìn)行PCR擴(kuò)增。結(jié)果 2011-2014年共分離21株MRSA,對(duì)15種臨床常用抗生素耐藥率分別為:青霉素100%,阿莫西林95.24%,苯唑西林95.24%,氨卡西林100%,四環(huán)素71.43%,克林霉素80.95%,慶大霉素85.71%,頭孢唑林100%,左氧氟沙星71.43%,莫西沙星38.1%,磺胺甲VA唑/甲氧芐啶42.86%,喹奴普汀/達(dá)福普汀4.76%,利福平38.10%,利奈唑胺4.76%,未檢出萬(wàn)古霉素耐藥菌株。21株MRSA中mecA、femA、16SrRNA和orfX基因檢出率均為100%,hla陽(yáng)性率為85.71%(18/21),icaA和clfB陽(yáng)性率均為80.95%(17/21),aac(6′)/aph(2′′)陽(yáng)性率為57.14%(12/21),hlg-2和pvl陽(yáng)性率均為76.19%(16/21),clfA陽(yáng)性率為71.43%(15/21),lukE陽(yáng)性率為66.67%(14/21),ermC陽(yáng)性率為52.38%(11/21),aac(3′)-Ⅲ陽(yáng)性率為47.62%(10/21),ermA陽(yáng)性率為28.57%(6/21),SasX和hlb陽(yáng)性率均為23.81%(5/21),未檢測(cè)到ermB基因。討論MRSA是醫(yī)院院內(nèi)感染常見(jiàn)致病菌,喹奴普汀/達(dá)福普汀、利奈唑胺和萬(wàn)古霉素可用于長(zhǎng)春地區(qū)MRSA感染的治療。
[Abstract]:Objective to investigate the detection of methicillin-resistant Staphylococcus aureus (MRSA) resistance and drug resistance genes in incision infection after thyroid surgery. Methods the specimens of incision infection after thyroid surgery in Changchun region from 2011 to 2014 were collected. The bacteria were identified by MicroScan WalkAway-40 automatic microbiological identification system of Siemens Company. The drug sensitivity test of MRSA was carried out by K-B method. Bacterial DNA was extracted and specific genes were designed for PCR amplification. Results A total of 21 MRSAs were isolated from 2011 to 2014. The resistance rates to 15 commonly used antibiotics were as follows: penicillin 100, amoxicillin 95.2424, oxacillin 95.24m, amoxicillin 100, tetracycline 71.43, clindamycin 80.955, gentamicin 85.71, cefazolin 100, levofloxacin 100. 71.43, moxifloxacin 38.1, sulfamethoxazole / trimethoprim 42.86, quinoplatin / dafopentin 4.76, rifampicin 38.1010, linazolamine 4.76. the positive rates of mecAfemAfemAmefen 16s rRNA and orfX gene in MRSA were 85.71% (18 / 21) and 85.71% (18 / 21), respectively.
