【摘要】：目的觀察醒腦靜注射液聯(lián)合床旁高流量持續(xù)血液凈化治療膿毒性休克毒熱內(nèi)盛證的療效,以及對血清腫瘤壞死因子(TNF-α)和神經(jīng)元特異性烯醇化酶(NSE)水平的影響。方法選擇符合條件病例共96例,參照隨機數(shù)字表法分為常規(guī)治療組和醒腦靜組,兩組均48例。除了基礎(chǔ)對癥治療,常規(guī)治療組給予床旁高流量持續(xù)血液凈化(CBP)治療;醒腦靜組在常規(guī)治療組基礎(chǔ)上給予醒腦靜注射液治療,兩組患者均連續(xù)治療1周。比較兩組的中醫(yī)主要癥狀評分、急性生理和慢性健康狀況評分Ⅱ(APACHEⅡ)和Marshall評分;檢測兩組血清TNF-α和NSE水平檢測。結(jié)果醒腦靜組治療后中醫(yī)主要癥狀評分高熱、煩躁、神昏、惡心嘔吐評分均顯著低于常規(guī)治療組(P0.01);醒腦靜組腦電圖療效總有效率為83.33%,常規(guī)治療組為60.42%,醒腦靜組明顯高于常規(guī)治療組(P0.05);治療后3d和1周,醒腦靜組APACHEⅡ和Marshall評分均顯著低于同期常規(guī)治療組(P0.01);治療后,醒腦靜組TNF-α和NSE水平均顯著低于常規(guī)治療組,比較差異有統(tǒng)計學(xué)意義(P0.01)。結(jié)論在西醫(yī)常規(guī)基礎(chǔ)治療上,醒腦靜注射液聯(lián)合床旁高流量CBP治療膿毒性休克毒熱內(nèi)盛證可明顯改善患者臨床癥狀,抑制患者血清TNF-α和NSE水平可能與上述治療效果密切相關(guān)。
[Abstract]:Objective to observe the effect of Xingnaojing injection (XNJI) combined with continuous blood purification with high flow rate in the treatment of hypertoxic heat syndrome in septic shock and its effect on serum tumor necrosis factor (TNF- 偽) and neuron-specific enolase (NSE) levels. Methods 96 cases were divided into routine treatment group and Xingnaojing group, 48 cases in both groups.
