【摘要】：目的運(yùn)用護(hù)理專案改善方法對(duì)ICU靜脈輸注胰島素患者的血糖管理進(jìn)行持續(xù)質(zhì)量改進(jìn),使重癥患者在靜脈輸注胰島素時(shí)安全、有效和平穩(wěn)降糖。方法回顧性分析2014年上半年ICU靜脈輸注胰島素患者低血糖發(fā)生率、血糖達(dá)標(biāo)時(shí)間、胰島素使用情況等,運(yùn)用專案改善方法制訂相應(yīng)胰島素輸注和血糖監(jiān)測質(zhì)控方案,對(duì)2014年下半年ICU靜脈輸注胰島素患者進(jìn)行血糖管理,對(duì)比對(duì)照組(上半年)和干預(yù)組(下半年)患者低血糖發(fā)生率、血糖達(dá)標(biāo)時(shí)間、胰島素使用劑量、開始和停止胰島素時(shí)血糖值。結(jié)果兩組ICU靜脈輸注胰島素患者低血糖發(fā)生率分別為10%和2%(P0.05);對(duì)照組和干預(yù)組患者平均一次靜脈輸注胰島素期間血糖達(dá)標(biāo)時(shí)間分別為(11.83±4.98)h和(10.15±5.34)h(P0.05);對(duì)照組和干預(yù)組患者平均一次靜脈輸注胰島素的劑量平均秩為54.96和44.94(P0.05);對(duì)照組和干預(yù)組患者開始使用胰島素時(shí)的血糖值分別為(18.70±3.70)mmol/L和(16.82±2.98)mmol/L(P0.05);對(duì)照組和干預(yù)組患者停止使用胰島素時(shí)的血糖值分別為(8.80±2.20)mmol/L和(9.88±2.31)mmol/L(P0.05)。結(jié)論通過專案改善,可降低ICU靜脈輸注胰島素患者的低血糖發(fā)生率,縮短血糖達(dá)標(biāo)時(shí)間,減少胰島素輸注劑量,提高護(hù)士預(yù)防低血糖的風(fēng)險(xiǎn)意識(shí)。
[Abstract]:Objective to improve the continuous quality of blood glucose management in patients with ICU intravenous insulin infusion by using the method of nursing project improvement, so as to make the severe patients safe, effective and stable when intravenous insulin is infused. Methods the incidence of hypoglycemia, the time of meeting the standard of blood glucose, and the use of insulin in patients with ICU intravenous insulin infusion in the first half of 2014 were analyzed retrospectively. In the second half of 2014, blood glucose management was performed in patients with ICU intravenous insulin infusion. The incidence of hypoglycemia, the time of blood glucose reaching the standard, and the dosage of insulin were compared between the control group (in the first half of the year) and the intervention group in the second half year. Blood glucose levels at the start and stop of insulin. Results the incidence of hypoglycemia was 10% and 2% (P0.05) in patients with insulin intravenous infusion of ICU, and the average time of meeting the standard of blood glucose in control group and intervention group was (11.83 鹵4.98) h and (10.15 鹵5.34) h (), respectively, while that in control group and intervention group was (11.83 鹵4.98) h and (10.15 鹵5.34) h (), respectively. Conclusion through special project improvement, the incidence of hypoglycemia, the time of blood glucose reaching the standard, the dosage of insulin infusion and the risk consciousness of nurses to prevent hypoglycemia can be reduced in patients with ICU intravenous insulin infusion.
