河南省德爾卑沙門菌和阿貢納沙門菌耐藥與分子分型研究

【摘要】:目的分析2009-2012年河南省德爾卑沙門菌和阿貢納沙門菌臨床分離株的耐藥狀況與脈沖場(chǎng)凝膠電泳(PFGE)帶型,為以德爾卑沙門菌、阿貢納沙門菌等非傷寒沙門菌為代表的食源性疾病暴發(fā)預(yù)警、調(diào)查、溯源及公共衛(wèi)生意義上的抗生素使用策略提供基線與參考數(shù)據(jù)。方法根據(jù)國際PulseNet細(xì)菌性傳染病分子分型監(jiān)測(cè)網(wǎng)絡(luò)公布的非傷寒沙門菌PFGE分型技術(shù)與美國臨床與實(shí)驗(yàn)室標(biāo)準(zhǔn)協(xié)會(huì)(CLSI)沙門菌K-B法藥敏測(cè)試方案,對(duì)2009-2012年分離自河南省5個(gè)哨點(diǎn)醫(yī)院的德爾卑沙門菌與阿貢納沙門菌進(jìn)行13種抗生素的藥敏測(cè)試及PFGE脈沖場(chǎng)凝膠電泳分子分型分析。結(jié)果 76株德爾卑沙門菌與84株阿貢納沙門菌對(duì)8類13種抗生素均有不同程度的耐藥,耐3種以上抗生素的有141株(88.13%)。其中耐3~5種抗生素38株(占23.75%),耐6~8種抗生素66株(占41.25%),耐9~10種抗生素22株(占13.75%),耐11~12種抗生素15株(占9.38%)。84株阿貢納沙門菌經(jīng)XbaⅠ酶切與脈沖場(chǎng)凝膠電泳后獲得36種帶型,每種帶型包含1~9個(gè)菌株,相似度50.77%~100%,以AGN27與AGN33為主要優(yōu)勢(shì)帶型。76株德爾卑沙門菌經(jīng)XbaⅠ酶切與脈沖場(chǎng)凝膠電泳后獲得41種帶型,每種帶型包含菌株數(shù)1~12個(gè)菌株,相似度43.25%~100%,以DER17與DER34為主要優(yōu)勢(shì)帶型。結(jié)論河南省臨床分離株德爾卑沙門菌和阿貢納沙門菌耐藥狀況較嚴(yán)重,PFGE帶型呈現(xiàn)出多樣性的同時(shí)又具有較顯著的優(yōu)勢(shì)帶型特點(diǎn),部分帶型與其對(duì)應(yīng)的耐藥譜具有一定的關(guān)聯(lián)性及相同的聚集性。
[Abstract]:Objective to analyze the drug resistance and pulsed field gel electrophoresis (PFGE) pattern of clinical isolates of Salmonella del Pei and Salmonella Argonella from 2009 to 2012 in Henan Province. Non-typhoid Salmonella, such as Salmonella Argonella, is the representative of food-borne disease outbreak early warning, investigation, traceability and public health sense of antibiotic use strategies to provide baseline and reference data. Methods according to the PFGE typing technique of non-typhoid Salmonella and the K-B drug sensitivity test program of the American Association of Clinical and Laboratory Standards (CLSI) published by the International PulseNet Monitoring Network for Bacteriological Infectious Diseases. Thirteen antibiotics of Salmonella delpezii and Salmonella Argonella isolated from 5 sentinel hospitals in Henan Province from 2009 to 2012 were tested for their drug sensitivity and molecular typing by PFGE pulsed field gel electrophoresis (PFGE). Results 76 strains of Salmonella delpezii and 84 strains of Argonella were resistant to 8 kinds of 13 kinds of antibiotics, 141 strains (88.13%) were resistant to more than 3 kinds of antibiotics. Among them, 38 strains (23.75%) were resistant to 5 kinds of antibiotics, 66 strains (41.25%) were resistant to 68 kinds of antibiotics, 22 strains (13.75%) were resistant to 9 or 10 kinds of antibiotics, 15 strains (9.38%) were resistant to 1112 kinds of antibiotics. Each band type consisted of 1 ~ 9 strains, and the similarity was 50.77100. With AGN27 and AGN33 as the main dominant bands, 41 bands were obtained by XBA 鈪,
