泛耐藥肺炎克雷伯菌耐藥基因檢測及聚類分析

【摘要】:目的調(diào)查三家醫(yī)院31株泛耐藥肺炎克雷伯菌(PDR-KPN)獲得性耐藥相關(guān)基因和可移動(dòng)遺傳元件遺傳標(biāo)記的存在狀況,并分析其親緣關(guān)系。方法收集三家醫(yī)院2013年1月-12月分離31株P(guān)DR-KPN;采用聚合酶鏈反應(yīng)(PCR)法檢測β-內(nèi)酰胺類、氨基糖苷類、喹諾酮類獲得性耐藥基因以及可移動(dòng)遺傳元件標(biāo)記基因。對(duì)全部耐藥基因檢測結(jié)果作樣本聚類分析。結(jié)果 31株P(guān)DR-KPN每株均檢出β-內(nèi)酰胺類、氨基糖苷類藥物獲得性耐藥基因,喹諾酮類藥物作用靶位gyrA基因突變陽性率100%;31株blaKPC與ISKpn6均陽性,blaKPC-ISKpn6連鎖檢測陽性率100.0%。樣本聚類分析顯示A、B、C三家醫(yī)院的菌株分為三個(gè)簇群。B醫(yī)院與C醫(yī)院的菌株有較近的親緣關(guān)系。結(jié)論本組菌株攜帶獲得性耐藥相關(guān)基因?qū)е聦?duì)相關(guān)抗菌藥物耐藥。指標(biāo)聚類分析提示菌株之間存在克隆傳播。
[Abstract]:Objective to investigate the existence of acquired resistance related genes and genetic markers of mobile genetic components of 31 drug-resistant Klebsiella pneumoniae (PDR-KPN) in three hospitals, and to analyze their genetic relationship. Methods 31 strains of PDR-KPN were isolated from three hospitals in January 2013 and 31 were isolated from -12 months, and beta lactam and aminoglycosides were detected by polymerase chain reaction (PCR). Quinolones acquired resistance gene and movable genetic element marker gene. The results of all drug resistance genes were analyzed by cluster analysis. Results of 31 PDR-KPN strains, beta lactam, aminoglycoside drug acquired resistance gene, and gyrA gene mutation positive rate of quinolone drug target were 100%, 31 strains of blaKPC and ISKp N6 positive, blaKPC-ISKpn6 linkage detection positive rate 100.0%. sample cluster analysis showed that the strains of three hospitals in A, B, C were divided into three cluster groups.B hospital and C hospital strains have closer relationship. Conclusion this group of strains carrying acquired resistance related genes lead to resistance to related antimicrobial agents. Index cluster analysis suggests that the strains exist among the strains. In cloned transmission.
