江蘇農(nóng)村地區(qū)結(jié)核桿菌基因分型及耐藥性分析

【摘要】:目的研究江蘇省農(nóng)村地區(qū)分離的結(jié)核分枝桿菌基因多態(tài)性,從而了解菌株基因型與耐藥之間的相關(guān)性。方法采用羅氏培養(yǎng)基分離來(lái)自結(jié)核病患者痰標(biāo)本中的結(jié)核分枝桿菌,進(jìn)行間隔區(qū)寡核苷酸分型(spacer oligonucleotide typing,Spoligotyping)和比例法進(jìn)行4種一線抗結(jié)核藥物的藥敏試驗(yàn)。經(jīng)統(tǒng)計(jì)學(xué)分析,P0.05為差異具有統(tǒng)計(jì)學(xué)意義。結(jié)果 547株結(jié)核分枝桿菌的Spoligotyping分型結(jié)果通過(guò)與Spol DB4.0數(shù)據(jù)庫(kù)比對(duì)后顯示,試驗(yàn)菌株被分為18個(gè)簇,57個(gè)基因型,以北京基因型菌株為主,占79.89%,非北京家族菌株包括T(T1、T2)、U、H、MANU2、LAM10-CAM和CAS-DELH1等型別,各占11.15%、0.91%、0.37%、0.91%、0.18%和0.18%,以及26個(gè)型別(35株菌)為"新型"。藥敏試驗(yàn)發(fā)現(xiàn),總耐藥率為21.94%,耐多藥率為7.68%;單藥耐藥中,主要以鏈霉素和異煙肼耐藥為主。經(jīng)χ2檢驗(yàn)和Fisher確切概率法分析發(fā)現(xiàn),北京基因型與非北京基因型菌株的耐多藥性方面差異具有顯著統(tǒng)計(jì)學(xué)意義。結(jié)論江蘇省農(nóng)村分離的結(jié)核分枝桿菌具有明顯的基因多態(tài)性,北京基因型為主要流行型,并且北京家族菌株與耐多藥之間具有一定的相關(guān)性。
[Abstract]:Objective to study the genetic polymorphisms of Mycobacterium tuberculosis isolated from rural areas of Jiangsu Province, and to understand the relationship between genotype and drug resistance. Methods Mycobacterium tuberculosis isolated from sputum samples of patients with tuberculosis was isolated from Roche medium. Spacer oligonucleotide typing and drug susceptibility tests were performed for four first-line antituberculotic drugs by means of spacer oligonucleotide typing and proportion method. By statistical analysis, P0.05 for the difference has statistical significance. Results the results of Spoligotyping of 547 strains of Mycobacterium tuberculosis were compared with Spol DB 4.0 database. The results showed that the tested strains were divided into 18 clusters and 57 genotypes, the majority of which were Beijing genotypes (79.89%). Each 11.15% 0.91% 0.370.37% and 0.18% 0.18% and 0.18%, as well as 26 types of 35 strains) are "new type". The drug sensitivity test showed that the total drug resistance rate was 21.94 and the multidrug resistance rate was 7.68.The main drug resistance in single drug resistance was streptomycin and isoniazid. 蠂 2 test and Fisher exact probability analysis showed that there was significant difference in multidrug resistance between Beijing genotype and non-Beijing genotype strain. Conclusion Mycobacterium tuberculosis isolated from rural areas of Jiangsu Province has obvious gene polymorphism, Beijing genotype is the main epidemic type, and there is a certain correlation between Beijing family strains and multi-drug resistance.
