【摘要】：目的探討應(yīng)用FOCUS-PDCA程序控制慢性傷口護(hù)理風(fēng)險(xiǎn)事件的效果。方法運(yùn)用FOCUS-PDCA程序(發(fā)現(xiàn)、組織、澄清、理解、選擇、計(jì)劃、實(shí)施、檢查和執(zhí)行9個(gè)步驟)分析慢性傷口護(hù)理過(guò)程的各個(gè)環(huán)節(jié),明確慢性傷口處理過(guò)程中的風(fēng)險(xiǎn)問(wèn)題,成立持續(xù)質(zhì)量改進(jìn)小組,規(guī)范慢性傷口處理中各項(xiàng)操作流程及風(fēng)險(xiǎn)預(yù)案,統(tǒng)一標(biāo)準(zhǔn)。結(jié)果實(shí)施FOCUS-PDCA程序前護(hù)理缺陷率7.5%,實(shí)施后無(wú)發(fā)生護(hù)理缺陷;實(shí)施前后患者滿意率分別為90%和99%,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。總結(jié)有效的成果給予標(biāo)準(zhǔn)化,將制定的慢性傷口護(hù)理流程納入造口傷口門(mén)診工作規(guī)范,將未解決或新出現(xiàn)的問(wèn)題進(jìn)一步持續(xù)改進(jìn)。結(jié)論 FOCUSPDCA程序遵循持續(xù)改進(jìn)的理念,運(yùn)用科學(xué)的方法解決質(zhì)量問(wèn)題,將調(diào)查問(wèn)題與解決問(wèn)題有效結(jié)合。我院規(guī)范了慢性傷口處理中各項(xiàng)操作流程及風(fēng)險(xiǎn)預(yù)案,統(tǒng)一標(biāo)準(zhǔn),有章可循,在保障患者安全的基礎(chǔ)上,也提升了傷口護(hù)理水平。
[Abstract]:Objective to investigate the effect of FOCUS-PDCA program on controlling risk events of chronic wound nursing. Methods the FOCUS-PDCA program (discovery, organization, clarification, understanding, selection, planning, implementation, inspection and execution of 9 steps) was used to analyze all aspects of the nursing process of chronic wound and to identify the risk problems in the process of chronic wound management. Set up continuous quality improvement team, standardize the operation process and risk plan in chronic wound management, unify the standard. Results the rate of nursing defect before FOCUS-PDCA program was 7.5, and there was no nursing defect after implementation, the satisfaction rate of patients before and after implementation was 90% and 99% respectively, the difference was statistically significant (P 0.05). The effective results were summarized and standardized, the established nursing process of chronic wound was brought into the outpatient work standard of open wound, and the unsolved or emerging problems were further improved. Conclusion FOCUSPDCA program follows the concept of continuous improvement and uses scientific methods to solve quality problems. Our hospital has standardized the operation flow and the risk plan in the chronic wound management, unifies the standard, has the rule to follow, in the safeguard patient's safety foundation, also has promoted the wound nursing level.
