【摘要】：目的對(duì)1例產(chǎn)后出血輸血后產(chǎn)生意外抗體進(jìn)行血型血清學(xué)分析,對(duì)患者輸血過(guò)程及輸血后發(fā)生溶血過(guò)程進(jìn)行監(jiān)管分析。方法采用血型血清學(xué)方法檢測(cè)輸血意外抗體,按《臨床輸血技術(shù)規(guī)范》監(jiān)管輸血過(guò)程,觀察輸血后患者Coomb,s試驗(yàn)、血、尿常規(guī)、肝、腎功能變化了解機(jī)體對(duì)溶血代謝。結(jié)果輸血產(chǎn)生意外抗體為抗-Ec聯(lián)合抗體,與本次妊娠無(wú)關(guān),與輸血不合紅細(xì)胞量及次數(shù)相關(guān),抗體生成逐漸增多,有明顯溶血癥狀是抗體效價(jià)≥8以后,溶血停止在輸入不合紅細(xì)胞全部破壞,診斷"紅細(xì)胞輸注無(wú)效",發(fā)生遲發(fā)性溶血輸血反應(yīng),配合DCCee紅細(xì)胞輸注,輸血效果顯著。結(jié)論加強(qiáng)臨床用血管理,及早發(fā)現(xiàn)輸血意外抗體,為患者提供恰當(dāng)劑量合適紅細(xì)胞制品,達(dá)到預(yù)計(jì)輸血效果,減少輸血意外抗體所致溶血輸血反應(yīng)。
[Abstract]:Objective to analyze the blood type serology of a patient with postpartum hemorrhage after transfusion, and to monitor the process of blood transfusion and hemolysis after transfusion. Methods Blood group serology was used to detect the blood transfusion accident antibody, and the blood transfusion process was supervised according to the Technical Specification of Clinical Blood Transfusion.
