【摘要】：目的:本研究擬運(yùn)用靜息態(tài)功能磁共振成像探討皮質(zhì)下腦卒中患者運(yùn)動(dòng)想象有關(guān)腦區(qū)功能連接狀況。方法:對(duì)24例病灶位于左側(cè)運(yùn)動(dòng)通路的皮質(zhì)下腦卒中患者和年齡、性別相匹配的24例正常對(duì)照進(jìn)行靜息態(tài)腦功能磁共振成像掃描,以病變側(cè)(左側(cè))頂下小葉為種子點(diǎn),計(jì)算它與其他腦區(qū)的功能連接,利用雙樣本t檢驗(yàn)對(duì)比分析兩組受試者靜息態(tài)功能連接差異。結(jié)果:與正常對(duì)照比較,腦卒中組的運(yùn)動(dòng)想象相關(guān)腦區(qū)之間功能連接發(fā)生顯著變化。腦卒中組病變側(cè)頂下小葉與病變對(duì)側(cè)腦功能連接顯著下降,涉及的腦區(qū)(頂下小葉、輔助運(yùn)動(dòng)皮質(zhì)、運(yùn)動(dòng)前區(qū)、額下回島蓋部和枕中回)多于病變同側(cè)(輔助運(yùn)動(dòng)皮質(zhì)和運(yùn)動(dòng)前區(qū));頂下小葉與病變同側(cè)的額下回島蓋部和雙側(cè)小腦外側(cè)部的功能連接顯著增加。結(jié)論:靜息態(tài)磁共振成像功能連接分析成功顯示皮質(zhì)下腦卒中導(dǎo)致運(yùn)動(dòng)想象相關(guān)腦區(qū)功能連接受損的客觀存在。
[Abstract]:Aim: to investigate the functional connection of motor imagination in patients with subcortical stroke using resting functional magnetic resonance imaging. Methods: Twenty-four patients with subcortical stroke whose lesions were located in the left motor pathway and 24 normal controls matched with age and sex were examined with resting functional magnetic resonance imaging (fMRI). The infra parietal lobules of the lesion side (left) were used as the seed point. The functional connections between the two groups and other brain regions were calculated, and the differences of resting functional connections between the two groups were analyzed by double sample t-test. Results: compared with the normal controls, the functional connections between the motor imagination related brain regions in the stroke group were significantly changed. In the stroke group, the functional connections between the lesion and the contralateral lobules were significantly decreased (subparietal lobules, auxiliary motor cortex, premotor area, premotor area). The inferior frontal gyrus and the middle occipital gyrus were more than the ipsilateral (auxiliary motor cortex and premotor area), and the functional connections between the inferior parietal lobule and the ipsilateral tegmental part of the inferior frontal gyrus and the lateral part of the bilateral cerebellum were significantly increased. Conclusion: resting magnetic resonance imaging functional connection analysis successfully demonstrated the existence of functional connection impairment in motor imagination related brain area after subcortical stroke.
