【摘要】：目的研究血漿過敏原篩查在預防輸血過敏反應中的臨床應用。方法回顧性分析住院輸血患者病例資料和患者血漿過敏原篩查報告,按過敏原陽性項目數,將輸血患者分為低敏體質組(Ⅰ組:患者血漿過敏原篩查結果≤3項陽性)和易敏體質組(Ⅱ組:患者血漿過敏原篩查結果≥5項陽性),選擇Ⅱ組中26例反復輸血過敏的血液科患者,在后續(xù)住院期間輸血前采取藥物預防措施。結果易敏體質患者(Ⅱ組)供輸血108次,出現過敏反應71次,發(fā)生率為65.7%(71/108),低敏體質組(I組)的患者供輸血513次,出現過敏反應17次,過敏反應發(fā)生率為3.3%(17/513);26例反復輸血過敏血液病患者輸血82次,過敏反應56次,過敏發(fā)生率68.3%,這些患者在后續(xù)住院期間,輸血前實施藥物預防過敏措施后輸血71次,過敏反應1次,過敏發(fā)生率1.4%(P0.01)。結論輸血患者在輸注血漿制品前進行血漿過敏原篩查,依據篩查的結果對易敏體質患者進行輸血過敏藥物預防,可有效降低輸血過敏反應的發(fā)生率。
