淋球菌對(duì)大觀霉素耐藥機(jī)制的研究

【摘要】:目的探討淋球菌16Sr RNA和rps E基因突變與淋球菌對(duì)大觀霉素耐藥性之間的關(guān)系。方法1.瓊脂稀釋法檢測(cè)6株淋球菌對(duì)大觀霉素的最小抑菌濃度(MIC)2.對(duì)臨床分離的4株大觀霉素耐藥的淋球菌株(MIC128mg/L、256mg/L)的16S r RNA和rps E基因進(jìn)行PCR擴(kuò)增測(cè)序分析,尋找可能的突變位點(diǎn).3.通過(guò)DNA轉(zhuǎn)化技術(shù)將含有突變的rps E基因的細(xì)菌基因組DNA轉(zhuǎn)化入敏感的淋球菌株,檢測(cè)轉(zhuǎn)化成功的淋球菌的MIC并進(jìn)行PCR擴(kuò)增測(cè)序,分析發(fā)現(xiàn)的突變位點(diǎn)與淋球菌大觀霉素耐藥的相關(guān)性。結(jié)果1.對(duì)大觀霉素敏感,耐藥者為4株、2株,2.4株大觀霉素耐藥的菌株均發(fā)現(xiàn)rps E基因A70C(Thr24Pro)突變,而16S r RNA大觀霉素耐藥決定區(qū)(SRDR)未發(fā)現(xiàn)任何突變;大觀霉素敏感菌株的16S r RNA和rps E基因均未發(fā)現(xiàn)突變。轉(zhuǎn)化后獲得的大觀霉素耐藥淋球菌中亦發(fā)現(xiàn)了同樣的rps E基因突變。結(jié)論淋球菌對(duì)大觀霉素耐藥菌株的耐藥性問(wèn)題已經(jīng)出現(xiàn),但是,全球范圍內(nèi)大觀霉素耐藥的案例還是非常罕見(jiàn)的。對(duì)廣譜頭孢菌素等抗菌藥物敏感性降低的多重耐藥淋球菌的出現(xiàn),使得大觀霉素仍然是目前世界上治療泌尿生殖系統(tǒng)淋球菌感染的有效藥物。本研究描述了核糖體5S蛋白中Thr24Pro突變可能影響其與16S r RNA 34螺旋的相互作用,從而導(dǎo)致大觀霉素耐藥。rps E等位基因中A70C單步突變與淋球菌大觀霉素耐藥性的形成有關(guān)。但本組研究耐藥菌數(shù)量較少,研究范圍有限,仍需進(jìn)一步對(duì)淋球菌耐藥性機(jī)制的研究。
[Abstract]:Objective to investigate the relationship between the mutations of 16Sr RNA and rps E genes in Neisseria gonorrhoeae and the resistance of Neisseria gonorrhoeae to spectinomycin.Method 1.The minimal inhibitory concentration of 6 strains of Neisseria gonorrhoeae to spectinomycin was detected by Agar dilution method.The 16s r RNA and rps E genes of four strains of gonorrhoeae resistant to spectinomycin were amplified and sequenced by PCR amplification.The genomic DNA of bacteria containing mutated rps E gene was transformed into a sensitive gonococcus strain by DNA transformation technique. The MIC of the transformed gonococcus was detected and sequenced by PCR amplification.The mutation site was found to be related to the resistance of Neisseria gonorrhoeae to spectinomycin.Result 1.4 strains were resistant to spectinomycin, and 4 strains were resistant to spectinomycin. All of them had the mutation of rps E gene A70CnThr24Pro. but no mutation was found in 16s r RNA.No mutation was found in 16s r RNA and rps E genes of Micromycin sensitive strains.The same mutation of rps E gene was also found in the transformed Neisseria gonorrhoeae.Conclusion the drug resistance of Neisseria gonorrhoeae to spectinomycin resistant strains has appeared, but the cases of resistance to spectinomycin in the world are very rare.The emergence of multidrug resistant Neisseria gonorrhoeae with decreased susceptibility to broad-spectrum cephalosporins makes spectinomycin still an effective drug for the treatment of gonococcal infection in the genitourinary system.This study described that Thr24Pro mutation in ribosomal 5S protein may affect its interaction with 16s r RNA 34 helix, leading to the single step mutation of A70C in the allele of spectinomycin resistance. RPS E allele is related to the formation of drug resistance of Neisseria gonorrhoeae.However, the number of drug-resistant bacteria in this study is relatively small and the scope of research is limited, so the mechanism of drug resistance of Neisseria gonorrhoeae needs to be further studied.
