患者治療教育在兒童特應(yīng)性皮炎長期治療管理中的作用

【摘要】:目的評估患者治療教育(therapeutic patient education,TPE)在兒童特應(yīng)性皮炎(atopic dermatitis,AD)長期治療管理中的作用,了解TPE對AD疾病嚴(yán)重程度、復(fù)發(fā)頻率以及對患者生活質(zhì)量的改善作用;評估以"AD教育課堂"為主的AD患者治療教育形式的可行性。方法北京兒童醫(yī)院皮膚科門診診治的298例2~12歲AD患兒按照隨機分組原則分為實驗組(intervention group,IG)和對照組(control group,CG),采用"AD教育課堂"的形式對實驗組患兒及其護理人員實施患者教育,隨訪期限6個月,分別在實驗開始時(T0)、3個月(T1)、6個月(T2)對患兒進行SCORAD評分評估并填寫兒童及幼兒皮膚病生活質(zhì)量指數(shù)調(diào)查問卷、皮膚護理-潤膚劑使用認知調(diào)查問卷等,以評估TPE在AD長期治療管理中的作用,判斷此教育形式的有效性及可行性。結(jié)果實驗組患兒的SCORAD評分、生活質(zhì)量(quality of life,QoL)均有顯著改善,護理人員對皮膚護理重要性的認知程度較前增加,實驗組評估結(jié)果明顯優(yōu)于對照組,差異有統(tǒng)計學(xué)意義。結(jié)論 TPE在AD患兒長期治療管理中發(fā)揮著重要作用,本研究采取的以"AD教育課堂"為主要形式的患者治療教育形式是可行的,其可積極推廣,但需不斷完善。
[Abstract]:Objective to evaluate the role of therapeutic patient education in the long-term management of atopic dermatitis (AD) in children with atopic dermatitis, and to understand the effect of TPE on the severity, recurrence frequency and quality of life of patients with AD.To evaluate the feasibility of treatment education for AD patients in AD education classroom.Methods 298 children with AD aged 12 years old were randomly divided into two groups: experimental group intervention group (IGG) and control group CGG. The experimental group and nursing staff were educated in the form of "AD education classroom".At the beginning of the experiment, the children were evaluated with SCORAD score and filling out the questionnaire of quality of life index of children and young children, and the questionnaire of skin care and skin moisturizer use cognitive survey, respectively, at the beginning of the experiment, the patients were assessed with SCORAD score and filled out the questionnaire of quality of life index of children and young children, respectively at the beginning of the experiment.To evaluate the role of TPE in the management of AD, and to evaluate the effectiveness and feasibility of this form of education.Results the SCORAD score and quality of life (QOL) of the children in the experimental group were significantly improved, and the cognition of the importance of skin care was increased. The evaluation results of the experimental group were obviously better than those of the control group, and the difference was statistically significant.Conclusion TPE plays an important role in the long-term management of patients with AD. The main form of treatment education in this study, "AD education classroom", is feasible, which can be actively popularized, but needs to be improved constantly.
