【摘要】：目的探討在靜脈補(bǔ)充常規(guī)劑量谷氨酰胺的基礎(chǔ)上,管飼補(bǔ)充一定劑量的谷氨酰胺對(duì)重癥胰腺炎(severe acute pancreatitis,SAP)患者血清蛋白水平改善有無(wú)疊加效應(yīng)。方法將92例SAP患者按隨機(jī)數(shù)字表法分成病例組和對(duì)照組,每組46例�；颊呔谌朐�48~72 h內(nèi)給予營(yíng)養(yǎng)支持,病例組靜脈常規(guī)補(bǔ)充谷氨酰胺雙肽20 g/L(谷氨酰胺13.25 g),再管飼營(yíng)養(yǎng)液(谷氨酰胺2.5 g/125 m L);對(duì)照組僅靜脈常規(guī)補(bǔ)充谷氨酰胺。觀察2組患者營(yíng)養(yǎng)支持前后的總蛋白(TP)、白蛋白(ALB)、前白蛋白(PA)、血紅蛋白(HGB)、總淋巴細(xì)胞計(jì)數(shù)(TLC)等指標(biāo)的變化情況。結(jié)果與對(duì)照組相比,病例組患者血清中ALB、PA、TLC明顯升高(P0.05),HGB降低明顯減少(P0.05)。結(jié)論在靜脈常規(guī)補(bǔ)充谷氨酰胺的基礎(chǔ)上,管飼一定劑量谷氨酰胺可以產(chǎn)生增加蛋白合成的疊加效應(yīng)。
[Abstract]:Objective to investigate the superposition effect of intravenously supplemented glutamine on serum protein level of severe acute patients with severe pancreatitis.Methods 92 patients with SAP were randomly divided into two groups (46 cases in each group).All the patients were given nutritional support within 48h after admission. The patients in the case group were given 20 g / L glutamine dipeptide (13.25 g / L glutamine 13.25 g / L) intravenously, and then intravenously supplemented with glutamine (2.5 g / 125 mL / L glutamine) in the control group.The changes of total protein (TPN), Albumin (ALB), prealbumin (PAA), hemoglobin (HGB) and total lymphocyte count (TLC) before and after nutritional support were observed in the two groups.Results compared with the control group, the serum levels of TLC in the case group were significantly higher than those in the control group, and the decrease of HGB was significantly lower than that of the control group.Conclusion on the basis of routine supplementation of glutamine intravenously, a certain dose of glutamine could increase protein synthesis.
