離體豬肝膽道內(nèi)射頻消融時局部膽汁溫度及膽道損傷情況

【摘要】:目的探討不同功率膽道內(nèi)射頻消融后離體豬肝局部膽汁溫度變化及膽道損傷情況。方法選取10個新鮮離體豬肝(膽囊及膽總管中上段完整保留),于DSA下將收集的人膽汁注入豬膽道系統(tǒng),并注入少量對比劑,然后將HabibTMEndoHPB射頻消融導(dǎo)管經(jīng)膽總管引入肝門區(qū)膽管內(nèi)并用縫線將膽總管與消融導(dǎo)管綁緊。10個豬肝消融參數(shù)分別為5 W(120s)、6 W(120s)、7 W(120s)、8 W(120s)、9 W(120s)、10 W(120s)、11 W(120s)、12 W(120s)、13 W(120s)、14 W(120s)。透視下分別于遠端消融電極、距遠端消融電極1、2cm處插入測溫針進行測溫。消融后取消融區(qū)膽管及毗鄰組織送病理檢查評價損傷程度。結(jié)果 10個人膽汁充盈離體豬肝膽道系統(tǒng)消融模型均成功建立,且成功導(dǎo)入射頻消融導(dǎo)管進行消融。測溫顯示消融區(qū)溫度隨功率增加而升高、最高達90.3℃;距遠端消融電極1、2cm處溫度上升不明顯,為28.4~40.2℃。病理檢查顯示消融區(qū)膽管出現(xiàn)全層凝固性壞死,距消融區(qū)1、2cm處膽管及毗鄰肝組織無明顯損傷。結(jié)論膽道內(nèi)消融可使鄰近膽道內(nèi)膽汁溫度有所升高,但對毗鄰非消融區(qū)膽管內(nèi)皮無明顯損傷,提示膽汁熱傳導(dǎo)作用對毗鄰非消融區(qū)膽管及肝組織影響較小。
