【摘要】：目的研究艱難梭菌感染患者的臨床特征,為臨床對(duì)此類感染患者的診斷及治療提供依據(jù)。方法回顧性分析感染性腹瀉患者的艱難梭菌分離情況以及臨床感染特點(diǎn)。結(jié)果從291份感染性腹瀉標(biāo)本中分離出33株艱難梭菌,陽(yáng)性率為10.3%(18/174),其中26株為產(chǎn)A/B毒素菌株(12例患者)。艱難梭菌主要分離自老年男性,均具有嚴(yán)重基礎(chǔ)疾病,且使用過質(zhì)子泵抑制劑和多種廣譜抗菌藥物;糞便常規(guī)和血液學(xué)相關(guān)指標(biāo)對(duì)艱難梭菌感染及產(chǎn)毒與否具有一定的指示作用。結(jié)論嚴(yán)重的艱難梭菌感染患者病死率高,且易引起院內(nèi)感染的播散,醫(yī)院可以根據(jù)此類患者的臨床特征進(jìn)行規(guī)范干預(yù),從而有效地控制院內(nèi)感染的爆發(fā)。
[Abstract]:Objective to study the clinical characteristics of patients infected with Clostridium difficulty. Methods A retrospective analysis was made on the isolation and clinical infection characteristics of Clostridium difficulty in patients with infectious diarrhea. Results 33 strains of Clostridium difficulty were isolated from 291 specimens of infectious diarrhea. The positive rate was 10.3 / 174%, of which 26 strains were A- / B toxin producing strains (12 cases). Clostridium diffusa was isolated mainly from old men, all of them had serious basic diseases and had been treated with proton pump inhibitors and a variety of broad-spectrum antimicrobial agents. The stool routine and hematological indexes can indicate the infection of Clostridium difficulty and whether it can produce poison or not. Conclusion the mortality rate of patients with severe Clostridium difficulty infection is high, and it is easy to cause the spread of nosocomial infection. Hospital intervention can be standardized according to the clinical characteristics of such patients to effectively control the outbreak of nosocomial infection.
