2010-2014年腸球菌屬臨床分布與耐藥性監(jiān)控

【摘要】:目的分析腸球菌屬感染的臨床分布及其耐藥性,為臨床合理使用抗菌藥物提供依據(jù)。方法收集2010-2014年醫(yī)院臨床分離腸球菌屬,應(yīng)用VITEK-2Compact微生物全自動分析系統(tǒng)進行鑒定,采用AST-GP67藥敏卡進行藥敏試驗,用WHONET5.6軟件分析腸球菌屬的分布及耐藥性。結(jié)果 2010-2014年共分離出482株腸球菌屬,其中糞腸球菌286株、屎腸球菌153株,主要來源于尿液、引流液和血液;糞腸球菌對氨芐西林、青霉素、紅霉素、環(huán)丙沙星、左氧氟沙星、莫西沙星、高濃度慶大霉素、高濃度鏈霉素、呋喃妥因、萬古霉素等10種抗菌藥物耐藥率分別為24.1%、19.2%、66.4%、18.5%、19.2%、19.2%、38.1%、30.4%、0.3%、0.3%,明顯低于屎腸球菌94.1%、92.2%、91.5%、93.5%、88.2%、89.5%、70.6%、49.0%、40.5%、2.6%,糞腸球菌對喹奴普汀/達福普汀、四環(huán)素耐藥率分別為100.0%、76.6%,明顯高于屎腸球菌0、42.5%;2010-2014年糞腸球菌及屎腸球菌對常用抗菌藥物耐藥率無明顯趨勢變化。結(jié)論糞腸球菌和屎腸球菌主要分布于尿液、引流液和血液,對多種抗菌藥物的耐藥率存在較大差異,臨床應(yīng)根據(jù)藥敏試驗結(jié)果合理選擇抗菌藥物。
[Abstract]:Objective to analyze the clinical distribution and drug resistance of Enterococcus infection in order to provide evidence for rational use of antimicrobial agents. Methods Clinical isolates of Enterococcus from 2010 to 2014 were collected and identified by VITEK-2Compact microorganism automatic analysis system. The distribution and drug resistance of Enterococcus were analyzed by WHONET5.6 software. Results A total of 482 strains of Enterococcus were isolated from 2010 to 2014, including 286 strains of Enterococcus faecalis and 153 strains of Enterococcus faecium, mainly from urine. Drainage fluid and blood; Enterococcus faecalis against ampicillin, penicillin, erythromycin, ciprofloxacin, levofloxacin, moxifloxacin, high concentration gentamicin, high concentration streptomycin, furantoin, The resistance rates of 10 antimicrobial agents, including vancomycin and so on, were 24.1and 19.2i, respectively, and the resistance rates were 66.45 and 19.219.2, respectively, and there were 38.1and 30.40.34 in the area, which were significantly lower than those of Enterococcus faecium 94.112.92. 2 and 993.588.28.28.50.75 and 49.00.45, respectively. Enterococcus faecalis had antiquinoputine / Dafoputin, and had a greater resistance to quinaputin / Dafopudine, which was significantly lower than that of Enterococcus faecium 94.51, 88.28.22, 89.50.6 and 49.00.45, and it was lower than that of Enterococcus faecium. The resistance rate of tetracycline to antibiotics was 100.0 and 76.6, respectively, which was significantly higher than that of Enterococcus faecium 02.5. There was no significant change in resistance rate of Enterococcus faecalis and Enterococcus faecium to common antimicrobial agents from 2010 to 2014. Conclusion Enterococcus faecalis and Enterococcus faecium are mainly distributed in urine, drainage fluid and blood. There is great difference in drug resistance rate of many kinds of antimicrobial agents, so it is necessary to select antimicrobial agents according to the results of drug sensitivity test in clinic.
