急診科護(hù)士情緒管理及臨床溝通能力的調(diào)查研究

【摘要】:目的調(diào)查急診科護(hù)士情緒管理及臨床溝通能力現(xiàn)狀,分析影響其情緒管理的因素,探討急診科護(hù)士情緒管理與臨床溝通能力之間的相關(guān)性,為護(hù)理管理者開(kāi)展臨床護(hù)士情緒管理培訓(xùn)、提升臨床護(hù)士溝通能力提供相關(guān)依據(jù)。方法選取2013年10~12月青島市三所三甲醫(yī)院182名急診科護(hù)士進(jìn)行問(wèn)卷調(diào)查。調(diào)查工具包括急診科護(hù)士的一般資料調(diào)查表、護(hù)士情緒管理量表、護(hù)士臨床溝通能力量表三部分。利用SPSS l7.0進(jìn)行數(shù)據(jù)統(tǒng)計(jì)分析,統(tǒng)計(jì)方法包括描述性分析、單因素分析、Spearman相關(guān)性分析及多元逐步回歸分析。結(jié)果1、急診科護(hù)士情緒管理現(xiàn)狀共發(fā)放調(diào)查問(wèn)卷200份,收回有效問(wèn)卷182份,有效回收率91%;急診科護(hù)士情緒管理處在中等水平以上,情緒管理總分(49.59±4.83)分,得分率為69%;四個(gè)維度按照其得分率的高低排序依次是情緒調(diào)整(14.20±1.93)分、情緒覺(jué)察(14.03±1.52)分、情緒運(yùn)用(13.83±1.82)分、情緒表達(dá)(7.52±1.23)分;2、急診科護(hù)士臨床溝通能力現(xiàn)狀調(diào)查示急診科護(hù)士臨床溝通能力處于中等以上水平,臨床溝通能力總分為(212.40±28.14)分,各維度得分由高到低分別為困難情景溝通能力(72.93±10.23)分、團(tuán)隊(duì)溝通能力(22.65±3.35)分、基本語(yǔ)言溝通能力(40.40±5.81)分、基本非語(yǔ)言溝通能力(25.60±3.84)分、情感支持能力(21.65±3.52)分、情感感知能力(29.16±4.81)分。3、急診科護(hù)士情緒管理影響因素分析單因素分析顯示不同性別、年齡、學(xué)歷、婚姻狀況、聘用形式對(duì)急診科護(hù)士情緒管理得分差異無(wú)統(tǒng)計(jì)學(xué)意義(P㧐0.05);不同護(hù)齡、職稱(chēng)、是否遭遇過(guò)工作中的暴力行為、工作倦怠及情緒轉(zhuǎn)移對(duì)急診科護(hù)士情緒管理得分差異有統(tǒng)計(jì)學(xué)意義(P㩳0.05);以急診科護(hù)士情緒管理為因變量,護(hù)齡、職稱(chēng)、是否遭遇過(guò)工作中的暴力行為、工作倦怠、情緒轉(zhuǎn)移以及臨床溝通能力進(jìn)行多元逐步回歸分析,最終工作倦怠、情緒轉(zhuǎn)移與臨床溝通能力三個(gè)變量進(jìn)入回歸方程,工作倦怠與情緒轉(zhuǎn)移回歸系數(shù)均為負(fù)值,說(shuō)明進(jìn)入方程的兩個(gè)變量對(duì)急診科護(hù)士情緒管理有負(fù)向預(yù)測(cè)作用;臨床溝通能力對(duì)急診科護(hù)士情緒管理有正向預(yù)測(cè)性;經(jīng)檢驗(yàn)回歸方程F為14.542,具有統(tǒng)計(jì)學(xué)意義(P㩳0.05)。4、急診科護(hù)士情緒管理與臨床溝通能力的相關(guān)性分析急診科護(hù)士情緒管理與臨床溝通能力呈正相關(guān),其中情緒調(diào)整維度、情緒運(yùn)用維度及情緒管理總分分別與臨床溝通能力各維度呈顯著正相關(guān)(P㩳0.01);而情緒表達(dá)維度相關(guān)系數(shù)最大為r=0.579,情緒覺(jué)察維度相關(guān)系數(shù)最大r=0.230。結(jié)論急診科護(hù)士情緒管理處于中等以上水平,建議應(yīng)將情緒管理引入護(hù)理管理領(lǐng)域,本著以人為本的管理理念做好情緒管理,進(jìn)一步提高臨床護(hù)理的服務(wù)與質(zhì)量。
[Abstract]:Objective to investigate the present situation of emotional management and clinical communication ability of nurses in emergency department, analyze the factors affecting their emotional management, and explore the correlation between emotional management and clinical communication ability of nurses in emergency department. To carry out emotional management training for clinical nurses for nursing managers, Methods 182 nurses in emergency department of three grade A hospitals in Qingdao city were investigated by questionnaire from October to December 2013. The survey tools included general information questionnaire of nurses in emergency department and nurses' emotional management scale. Three parts of nurses' clinical communication ability scale. Using SPSS L7.0 to carry on the statistical analysis, the statistical method includes descriptive analysis, Single factor analysis, Spearman correlation analysis and multiple stepwise regression analysis. Results 1. A total of 200 questionnaires were distributed and 182 valid questionnaires were collected. The effective recovery rate was 91%, the total score of emotion management was 49.59 鹵4.83) and the score rate was 690.The order of the four dimensions was emotion adjustment 14.20 鹵1.93), emotion perception 14.03 鹵1.52), emotion application 13.83 鹵1.82). The present situation of clinical communication ability of nurses in emergency department was found to be above the average level, and the total score of clinical communication ability was 212.40 鹵28.14. The scores from high to low were 72.93 鹵10.23, 22.65 鹵3.35, 40.40 鹵5.81, 25.60 鹵3.84 and 21.65 鹵3.52, respectively. Emotion perception ability was 29.16 鹵4.81) score. 3. Single factor analysis showed that there was no significant difference between gender, age, educational background, marital status, employment form and emotional management score of emergency department nurses. 0. 05%; different nursing years and professional titles, have you ever encountered violence at work, job burnout and emotional transfer have significant differences in emotional management scores of nurses in the emergency department (P < 0. 05)? Using emotional management of emergency department nurses as dependent variables, nursing age, professional title, whether or not they have experienced violence in work, job burnout, emotional transfer and clinical communication ability were analyzed by multivariate stepwise regression analysis, and finally job burnout was obtained. Three variables of emotional transfer and clinical communication ability entered the regression equation, and the regression coefficients of job burnout and emotional transfer were all negative, indicating that the two variables entered into the equation had a negative predictive effect on the emotional management of the nurses in the emergency department. The clinical communication ability had positive predictive effect on the emotional management of the nurses in the emergency department, and the regression equation F was 14.542, which had statistical significance. Correlation between emotional Management and Clinical Communication ability of Nurses in Emergency Department; emotional Management and Clinical Communication ability of Nurses in Emergency Department were positively correlated, among which emotional adjustment dimension, Emotional use dimension and total score of emotion management were significantly positively correlated with each dimension of clinical communication ability (P < 0.05). The correlation coefficient of emotion expression dimension and emotion awareness dimension was the highest, r = 0.579 and r = 0.230.Conclusion the emotional management of nurses in emergency department is above the middle level, and it is suggested that the emotional management should be introduced into the field of nursing management. In line with the people-oriented management concept to do a good job in emotional management, and further improve the clinical nursing service and quality.
