【摘要】：目的以?shī)W馬哈系統(tǒng)為基礎(chǔ)構(gòu)建符合我國(guó)國(guó)情的全髖關(guān)節(jié)置換術(shù)后患者從醫(yī)院到社區(qū)的一體化隨訪模式的效果。方法結(jié)合我國(guó)醫(yī)院、社區(qū)現(xiàn)狀制訂醫(yī)院轉(zhuǎn)介社區(qū)流程,以?shī)W馬哈系統(tǒng)為框架制訂社區(qū)隨訪流程及隨訪表,選擇2014年1月—2015年6月厚街醫(yī)院收治并行全髖關(guān)節(jié)置換術(shù)、術(shù)后轉(zhuǎn)介洪梅、厚街社區(qū)的患者63例,通過3次電話隨訪及1次家居隨訪對(duì)全部患者進(jìn)行術(shù)后10項(xiàng)常見護(hù)理問題的前瞻性干預(yù),并觀察其效果。結(jié)果通過隨訪干預(yù),患者生理領(lǐng)域的疼痛、神經(jīng)-肌肉-骨骼功能、排便、排尿功能的認(rèn)知、行為、狀況在出院1個(gè)月末得到顯著改善(P0.05);健康相關(guān)行為領(lǐng)域的營(yíng)養(yǎng)、睡眠休息形態(tài)、自我照顧的認(rèn)知、行為、狀況在出院3個(gè)月末得到改善(P0.05);出院后1個(gè)月末突顯社會(huì)心理問題,但隨著隨訪次數(shù)的增加,社會(huì)心理問題的認(rèn)知、行為、狀況評(píng)分(KBS評(píng)分)增加;家居隨訪對(duì)環(huán)境領(lǐng)域的住所改善效果優(yōu)于電話隨訪。結(jié)論醫(yī)院—社區(qū)一體化隨訪模式符合我國(guó)現(xiàn)有醫(yī)療體制狀況,能夠?qū)⑨t(yī)療資源分配最優(yōu)化;奧馬哈系統(tǒng)能夠切實(shí)有效地指導(dǎo)全髖關(guān)節(jié)置換術(shù)后患者的社區(qū)隨訪實(shí)踐,能夠分析患者的護(hù)理問題,改善狀況,動(dòng)態(tài)監(jiān)測(cè)結(jié)局變化,值得推廣應(yīng)用。
[Abstract]:Objective to establish an integrated follow-up model from hospital to community after total hip arthroplasty based on Omaha system. Using the Omaha system as the framework, the community follow-up procedure and the follow-up table were established. 63 patients were selected from January 2014 to June 2015 to receive total hip replacement in Houjie Hospital. 63 patients were referred to Hongmei and Houjie community after operation. Through three telephone follow-up and one home follow-up, all patients were treated with prospective intervention of 10 common nursing problems after operation, and their effects were observed. Results through follow-up intervention, pain in physiological field, neuromusculoskeletal function, and neuromusculoskeletal function were observed in all patients. The cognition, behavior and status of defecation and urination were significantly improved at the end of one month after discharge. At the end of 3 months after discharge, the condition was improved (P 0.05), and the psychosocial problems were highlighted at the end of one month after discharge, but with the increase of follow-up times, the scores of cognition, behavior and state of psychosocial problems increased. The effect of home follow-up was better than that of telephone follow-up in the environmental field. Conclusion the hospital community integrated follow-up model is in line with the existing medical system in China and can optimize the allocation of medical resources. Omaha system can effectively guide the community follow-up practice of patients after total hip arthroplasty, can analyze the nursing problems of patients, improve the situation, dynamically monitor the outcome changes, and it is worth popularizing and applying.
