【摘要】：目的探討1例稀有血型結(jié)腸癌患者緊急搶救輸血時(shí)因不規(guī)則抗體存在導(dǎo)致交叉配血困難而采用特殊情況緊急搶救輸血推薦方案的可行性。方法標(biāo)本首次血型檢測(cè)采用全自動(dòng)血型儀檢測(cè)血型(微孔板法),并用試管法復(fù)查患者血型;微柱凝膠抗球蛋白法、鹽水法、聚凝胺法、試管抗人球蛋白法做抗體篩查和交叉配血。結(jié)果患者血型為A型,Rh表型為ccDEE型,血清中含有抗-C和抗-e不規(guī)則抗體;經(jīng)過(guò)Rh表型篩選,從O型供者中找到Rh表型為ccDEE懸浮紅細(xì)胞1 U,從浙江省血液中心的A型供者中篩查到Rh表型為ccDEE懸浮紅細(xì)胞3 U,輸注后12 h檢測(cè)血紅蛋白61 g/L,提示輸注有效。有效地緩解了患者的貧血癥狀,無(wú)輸血不良反應(yīng)。結(jié)論對(duì)于因不規(guī)則抗體存在導(dǎo)致配血困難而又需緊急輸血搶救的腫瘤患者,在ABO血型同型無(wú)法滿足供應(yīng)時(shí),可篩選O型Rh表型一致且交叉配血試驗(yàn)陰性的供者紅細(xì)胞輸注。
[Abstract]:Objective to investigate the feasibility of using the recommended scheme of emergency rescue transfusion in a case of colon cancer with rare blood group because of the difficulty of cross matching caused by irregular antibody. Methods the blood group of a sample was detected for the first time. The blood group was detected by automatic blood type instrument (micropore plate method, and the blood group of patients was re-examined by test tube method). Microcolumn gel anti-globulin method, brine method, polyagglutination method, and test tube anti-human globulin method were used for antibody screening and cross-matching. Results the blood group of the patients was ccDEE phenotype, and there were anti-C and anti-e irregular antibodies in serum. After Rh phenotypic screening, The Rh phenotype was found to be ccDEE suspended erythrocyte 1U in O donors, and the Rh phenotype was found to be ccDEE suspended erythrocyte 3U from A donors in Zhejiang Blood Center. Hemoglobin 61g / L was detected 12 hours after infusion, indicating that the transfusion was effective. Effectively alleviated the patient's anemia symptoms, Conclusion for the tumor patients who have difficulty in matching blood caused by irregular antibodies and need emergency blood transfusion, when the ABO blood group is homotypic can not meet the supply, Donors with identical O type Rh phenotype and negative cross matching test could be screened for erythrocyte transfusion.
