IVIM-DWI成像鑒別兔軟組織VX2腫瘤凝固性壞死價值的探討

【摘要】:目的確定軟組織腫瘤內(nèi)有無壞死區(qū)對其生物學(xué)特性及治療效果的評估都有著重要的意義。腫瘤內(nèi)的壞死可為液化性壞死或凝固性壞死,對后者的MRI研究少見。本研究探討體素內(nèi)不相干運動彌散加權(quán)成像(intravoxel incoherent motion diffusion-weighted imaging,IVIM-DWI)在鑒別兔軟組織VX2腫瘤中存活組織與凝固性壞死組織的價值。方法在15只新西蘭兔左側(cè)大腿肌肉內(nèi)接種VX2腫瘤,3周后對其行動態(tài)對比劑增強磁共振成像(dynamic contrastenhanced MR imaging,DCE-MRI)和IVIM-DWI成像檢查(7個b值,0~800s/mm2)。采用MITK Diffusion軟件對IVIM-DWI數(shù)據(jù)進行處理,形成灌注分數(shù)(f值)和真性彌散系數(shù)(D值)的參數(shù)圖。MRI檢查完畢后處死實驗用兔獲取腫瘤標本,在軸位方向上切片,獲取與MR軸位圖像相對應(yīng)的大體標本斷層組織厚片,并以其為參照,并結(jié)合增強早期DCE-MRI在標本對應(yīng)層面的感興趣區(qū)(region of interest,ROI)測量存活腫瘤組織、凝固性壞死組織的f值和D值。分別采用Wilcoxon秩和檢驗和獨立樣本t檢驗比較存活腫瘤組織與凝固性壞死組織之間D值和f值的差異。結(jié)果所有實驗用兔的VX2腫瘤均種植成功。共選取51個ROI,其中存活腫瘤組織34個,凝固性壞死組織17個。存活腫瘤組織和凝固性壞死組織的f值分別為(7.995±2.867)%和(5.918±2.709)%,t=2.484,P=0.016。存活腫瘤組織與凝固性壞死組織的D值分別為(1.110±0.110)×10-3和(1.132±0.208)×10-3 mm2/s,Z=0.100,P=0.920。結(jié)論由IVIM-DWI所生成的f值能很好地鑒別兔軟組織VX2腫瘤中的存活腫瘤組織與凝固性壞死組織,而D值不能鑒別。
[Abstract]:Objective to determine whether there are necrotic areas in soft tissue tumors and to evaluate their biological characteristics and therapeutic effects. Necrosis in soft tissue tumors can be liquefaction necrosis or coagulative necrosis. MRI studies of the latter are rare. In this study, diffusion-weighted imaging of incoherent motion in voxels (. Intravoxel incoherent motion diffusion-weighted imaging. The value of IVIM-DWI in differentiating survival tissue from coagulant necrotic tissue in rabbit soft tissue VX2 tumor. Methods VX2 tumor was inoculated into the left thigh muscle of 15 New Zealand rabbits. After 3 weeks, dynamic contrast agent enhanced dynamic contrastenhanced Mr imaging was performed. Wilcoxon rank sum test and independent sample t test were used to compare the difference of D value and f value between viable tumor tissue and coagulative necrotic tissue. All the VX2 tumors were successfully implanted in rabbits. 51 ROI were selected. 34 of them survived tumor tissue. There were 17 coagulant necrotic tissues. The f values of surviving tumor tissues and coagulative necrotic tissues were 7.995 鹵2.867% and 5.918 鹵2.709%, respectively. The D values of survival tumor tissue and coagulative necrosis tissue were 1.110 鹵0.110 脳 10-3 and 1.132 鹵0.208, respectively. 脳 10-3 mm2/s. Conclusion the f value generated by IVIM-DWI can distinguish survival tumor tissue from coagulant necrotic tissue in rabbit soft tissue VX2 tumor. The D value could not be distinguished.
