李鐵教授養(yǎng)陰解毒法治療特發(fā)性血小板減少性紫癜的經(jīng)驗(yàn)

【摘要】：特發(fā)性血小板減少性紫癜(idiopathic thrombocytopenic purpura,ITP)是一組免疫介導(dǎo)的血小板破壞過多而導(dǎo)致的出血性疾病,其臨床表現(xiàn)主要有廣泛的皮膚黏膜和內(nèi)臟出血、血小板減少、骨髓巨核細(xì)胞發(fā)育成熟障礙、出血時間延長、及血小板膜糖蛋白特異性自身抗體出現(xiàn)等。在中醫(yī)上可將本病歸屬“血證”、“發(fā)斑”、“肌衄”等范疇。李鐵教授認(rèn)為“熱”、“瘀”、“濕”、“虛”是本病發(fā)生及進(jìn)展過程中的主要病機(jī)變化,運(yùn)用養(yǎng)陰解毒法治療陰虛型的血小板減少性紫癜,并提出了“兼顧清熱涼血,配伍活血散血,不離醒脾化濕”的養(yǎng)陰解毒原則。以四草四根湯為基礎(chǔ)方,配伍養(yǎng)陰、清熱、解毒、祛瘀、化濕等中藥。選取門診27例符合診斷標(biāo)準(zhǔn)的ITP患者,觀察李鐵教授養(yǎng)陰解毒法治療ITP的臨床療效。
[Abstract]:Idiopathic Thrombocytopenic Purpura (idiopathic thrombocytopenic purpura,ITP) is a group of hemorrhagic diseases caused by immune-mediated platelet destruction. Its clinical manifestations are extensive skin, mucous membrane and visceral bleeding, and thrombopenia. Bone marrow megakaryocytes developed and matured, bleeding time prolonged, and platelet membrane glycoprotein specific autoantibodies appeared. In traditional Chinese medicine, the disease can be classified into "blood syndrome", "plaque", "muscle bleeding" and other categories. Professor Li Tie believes that "heat", "blood stasis", "dampness" and "deficiency" are the main pathogenesis changes in the occurrence and progress of the disease. The method of nourishing yin and detoxification is used to treat thrombocytopenic purpura of yin deficiency type, and he put forward that "clearing away heat and cooling blood" The principle of nourishing yin and detoxification is compatible with promoting blood circulation and dispersing blood, not leaving the spleen to dissipate dampness. Based on Sicao Sigan decoction, it is compatible with traditional Chinese medicine such as nourishing yin, clearing away heat, detoxifying, removing blood stasis, removing dampness and so on. 27 patients with ITP who met the diagnostic criteria were selected to observe the clinical effect of Professor Li Tie's method of nourishing yin and detoxifying in the treatment of ITP.
