【摘要】：目的:通過(guò)臨床研究觀察加味清心蓮子飲治療中老年再發(fā)性尿路感染的臨床療效,并探討其作用機(jī)制,為臨床中西醫(yī)結(jié)合治療中老年人再發(fā)性尿路感染提供新方案和新思路。方法:將符合入選標(biāo)準(zhǔn)的自2014年4月至2015年12月在山東中醫(yī)藥大學(xué)附屬醫(yī)院及山東省立醫(yī)院腎內(nèi)科住院的患者60例,隨機(jī)分為2組,試驗(yàn)組和對(duì)照組各30例。對(duì)照組給予西藥抗生素分期減量治療方法,試驗(yàn)組在對(duì)照組的基礎(chǔ)上予加味清心蓮子飲。療程12個(gè)周,隨訪6周。分別觀察治療前后中醫(yī)證候總積分、尿白細(xì)胞、尿紅細(xì)胞及尿培養(yǎng)的變化。結(jié)果:共有59例患者完成12周臨床觀察。(1)臨床總有效率:試驗(yàn)組臨床治愈17人(56.67%)、有效9人(30%)、無(wú)效4人(13.33%),總有效率為86.67%;對(duì)照組臨床治愈11人(37.93%)、有效7人(24.14%)、無(wú)效11人(37.93%),總有效率為62.07%。兩組比較,試驗(yàn)組療效明顯優(yōu)于對(duì)照組(P0.05)。(2)尿常規(guī)變化:治療后尿白細(xì)胞、紅細(xì)胞檢查較治療前顯著下降,試驗(yàn)組較對(duì)對(duì)照組下降更為明顯(P0.05)。(3)中醫(yī)證候療效:試驗(yàn)組臨床治愈18人(60%)、顯效5人(16.67%)、有效2人(6.67%),無(wú)效5人(16.67%),總有效率為83.34%;對(duì)照組臨床治愈6人(20.69%)、顯效5人(17.24%)、有效2人(6.90%)、無(wú)效16人(55.17%),總有效率為44.83%。兩組比較,試驗(yàn)組療效優(yōu)于對(duì)照組(P0.05)。結(jié)論:加味清心蓮子飲聯(lián)合應(yīng)用抗生素治療中老年再發(fā)性尿路感染對(duì)減輕患者的臨床癥狀、降低復(fù)發(fā)率具有明顯的作用。
[Abstract]:Objective: to observe the clinical effect of Jiawei Qingxin Lianzi decoction in the treatment of recurrent urinary tract infection in the middle and old age, and to explore its mechanism, so as to provide a new scheme and new idea for the clinical treatment of recurrent urinary tract infection in the middle aged and the elderly. Methods: from April 2014 to December 2015, 60 patients hospitalized in the affiliated Hospital of Shandong University of traditional Chinese Medicine and Shandong Provincial Hospital were randomly divided into two groups: experimental group (n = 30) and control group (n = 30). The control group was treated with western medicine antibiotics by stages, and the experimental group was given Jiawei Qingxinlian seed drink on the basis of the control group. The course of treatment was 12 weeks and followed up for 6 weeks. The changes of total score of TCM syndromes, urine white blood cells, urine red blood cells and urine culture were observed before and after treatment. Results: a total of 59 patients completed 12-week clinical observation. (1) Clinical total effective rate: 17 patients (56.67%) were cured, 9 patients (30%) were effective, 4 patients (13.33%) were ineffective, and the total effective rate was 86.67%. In the control group, 11 cases (37.93%) were cured, 7 cases (24.14%) were effective, 11 cases (37.93%) were ineffective, and the total effective rate was 62.07%. Compared with the two groups, the curative effect of the experimental group was significantly better than that of the control group (P 0.05). (2). The changes of urine leukocytes and red blood cells after treatment were significantly lower than those before treatment. The therapeutic effect of TCM syndrome in the test group was more obvious than that in the control group (P0.05). (3): 18 cases (60%), 5 cases (16.67%), 2 (6.67%), 5 (16.67%), 5 (16.67%) were cured, 5 (16.67%), 5 (16.67%), respectively. The total effective rate was 83. 34%. In the control group, 6 cases (20.69%) were cured, 5 cases (17.24%) were markedly effective, 2 cases (6.90%) were effective, 16 cases (55.17%) were ineffective, and the total effective rate was 44.83%. Compared with the two groups, the therapeutic effect of the experimental group was better than the control group (P0.05). Conclusion: Jiawei Qingxin Lianzi decoction combined with antibiotics in the treatment of recurrent urinary tract infection in middle-aged and elderly patients has obvious effect on alleviating the clinical symptoms and reducing the recurrence rate.
