TACE聯(lián)合氬氦刀冷凍治療對(duì)肝癌血清血管內(nèi)皮生長(zhǎng)因子的影響

【摘要】：目的對(duì)原發(fā)性肝癌患者分別采用TACE (transcatheter arterial chemoemboli zat ion, TACE)治療和TACE聯(lián)合氬氬刀冷凍消融(argon-helium cryoablation therapy, AHC)治療,分別檢測(cè)患者治療前后血清中血管內(nèi)皮生長(zhǎng)因子(vascular endothelial growth factor, VEGF)水平,研究2種治療方法對(duì)肝癌患者血清中血管內(nèi)皮生長(zhǎng)因子(VEGF)表達(dá)水平影響。方法 收集湖南省人民醫(yī)院介入血管外科從2013年3月至2014年3月期間50例原發(fā)性肝癌患者,分別行TACE治療(對(duì)照組,n=25)和氬氦刀聯(lián)合TACE治療組(實(shí)驗(yàn)組,n=25)。分別在治療前的第1d,治療后的第1d、第3d、第7d、第28d,抽取靜脈血測(cè)定VEGF表達(dá)水平。結(jié)果 (1)TACE治療組血清VEGF水平的檢測(cè)：TACE組(對(duì)照組)術(shù)前為286.04pg/ml,術(shù)后第1d、3d、7d、28d的血清VEGF值分別為547.04、411.04、313.87、301.38pg/ml。TACE術(shù)后肝癌患者的血清VEGF表達(dá)水平呈現(xiàn)先升高后降低的動(dòng)態(tài)變化趨勢(shì),術(shù)后第1d VEGF表達(dá)水平最高,與術(shù)前比較差異有統(tǒng)計(jì)學(xué)意義(P0.05) 術(shù)后第3、7d肝癌患者血清中VEGF水平較術(shù)后1d降低,與術(shù)前比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)；術(shù)后第28d較術(shù)前VEGF表達(dá)水平仍稍高,但差異無統(tǒng)計(jì)學(xué)意義(P0.05)。(2) TACE聯(lián)合氬氦刀治療組(實(shí)驗(yàn)組)術(shù)前為283.69pg/ml,術(shù)后第1d、3d、7d、28d的血清VEGF值分別為509.42、380.01、194.64、279.85pg/ml,聯(lián)合治療后肝癌患者的VEGF表達(dá)水平呈現(xiàn)先升高后降低再升高的動(dòng)態(tài)變化趨勢(shì),術(shù)后第1、3d VEGF表達(dá)水平升高,與術(shù)前比較差異有統(tǒng)計(jì)學(xué)意義(P0.05) 術(shù)后第7d肝癌患者血清中VEGF水平較術(shù)前降低明顯,差異有統(tǒng)計(jì)學(xué)意義(P0.05)；術(shù)后第28d VEGF表達(dá)水平與術(shù)前比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。(3)聯(lián)合治療組術(shù)后各個(gè)時(shí)間點(diǎn)與TACE治療組比較,血清中VEGF的水平均較低,且差異均有統(tǒng)計(jì)學(xué)意義(P0.05),術(shù)后7d聯(lián)合組較TACE組有明顯下降趨勢(shì)。結(jié)論(1) TACE治療原發(fā)性肝癌,患者血清中VEGF表達(dá)水平升高;(2) TACE聯(lián)合氬氦刀冷凍治療原發(fā)性肝癌后,患者血清中VEGF的表達(dá)水平比TACE治療要低；(3) TACE聯(lián)合氬氦刀冷凍治療要優(yōu)于單純TACE治療。
[Abstract]:Objective to detect the level of vascular endothelial growth factor (vascular endothelial growth factor, VEGF) in serum of patients with primary liver cancer before and after treatment with TACE (transcatheter arterial chemoemboli zat ion, TACE) and TACE combined with argon ablation (argon-helium cryoablation therapy, AHC). To study the effect of two treatment methods on the expression of vascular endothelial growth factor (VEGF) in serum of patients with liver cancer. Methods from March 2013 to March 2014, 50 patients with primary liver cancer in the interventional vascular surgery of Hunan Provincial people's Hospital were treated with TACE (control group, nong25) and argon-helium combined with TACE treatment group (experimental group, nong25). The expression of VEGF was measured on the first day before treatment, on the first day, on the third day, on the seventh day and on the 28th day after treatment. Results (1) the level of serum VEGF in TACE group was 286.04pg / ml before operation in TACE group (control group), and the serum VEGF level of TACE group (control group) was increased first and then decreased after 547.04411.04313.87301.38pg/ml.TACE operation. The expression of VEGF was the highest on the 1st day after operation, which was significantly different from that before operation (P0.05). The level of serum VEGF in patients with liver cancer on the 3rd day after operation was lower than that on the 1st day after operation (P0.05). On the 28th day after operation, the expression of VEGF was still slightly higher than that before operation. However, there was no significant difference (P0.05). (2) between TACE and argon-helium therapy group (experimental group) was 283.69 PG / ml before operation, and the serum VEGF value on the 1st day after operation was 509.42380.01 194.279.85 pg-1 路ml, respectively. After combined therapy, the VEGF expression level of HCC patients increased first and then decreased, and then increased. The expression of VEGF increased on the 1st day after operation, which was significantly higher than that before operation (P0.05). The level of serum VEGF in patients with liver cancer on the 7th day after operation was significantly lower than that before operation (P0.05). There was no significant difference in the expression of VEGF between the two groups on the 28th day after operation (P0.05). (3). Compared with the TACE group, the serum VEGF level in the combined treatment group was lower than that in the TACE group at all time points after operation (P0.05). (3). And the difference was statistically significant (P0.05), 7 days after the combined group than the TACE group has a significant downward trend. Conclusion (1) the expression of VEGF in serum of patients with primary liver cancer treated with TACE is higher than that of patients with primary liver cancer, (2) the expression of VEGF in serum of patients with primary liver cancer treated with TACE combined with cryotherapy with argon-helium knife is lower than that with TACE. (3) TACE combined with argon-helium cryotherapy is superior to TACE alone.

(3) TACE combined with argon-helium cryotherapy is superior to TACE alone.
