直腸癌放療不同技術和體位對小腸受照劑量的影響

【摘要】:目的探討不同直腸癌放療技術對小腸受照劑量的影響。方法選擇2015年3月~2018年3月在我院診斷治療的直腸癌患者31例為研究對象。所有患者照射方案包括俯臥位3D-CRT、仰臥位3D-CRT、俯臥位IMRT與仰臥位IMRT。比較兩種治療體位PTV、小腸、膀胱照射體積,比較相同治療體位兩種照射技術小腸受照體積及相同照射技術不同體位小腸受照體積。結果兩組體位靶區(qū)PTV與小腸照射體積比較差異無統(tǒng)計學意義(P0.05);俯臥位膀胱照射體積顯著小于仰臥位,差異有統(tǒng)計學意義(P0.05)。俯臥位3D-CRT放療技術小腸V45受照體積顯著高于IMRT,差異有統(tǒng)計學意義(P0.05)。仰臥位3D-CRT放療技術小腸V45受照體積顯著高于IMRT,差異有統(tǒng)計學意義(P0.05)。3D-CRT技術下仰臥位小腸V45顯著高于俯臥位,差異有統(tǒng)計學意義(P0.05)。IMRT放療技術下仰臥位小腸V45顯著高于俯臥位,差異有統(tǒng)計學意義(P0.05)。結論相同的體位,IMRT相對于3DCRT小腸照射劑量更少,而相同的照射技術下,俯臥位小腸V45更小,提示俯臥位照射能夠減少小腸的受照劑量。
[Abstract]:Objective to investigate the effects of different radiotherapy techniques on small intestine irradiation dose in rectal cancer. Methods 31 patients with rectal cancer diagnosed and treated in our hospital from March 2015 to March 2018 were selected. All patients were treated with 3D-CRT in prone position, 3D-CRT in supine position, IMRT in prone position and IMRT. in supine position. The small intestine irradiation volume and bladder irradiation volume of PTV, were compared between the two treatment positions, the small intestinal irradiation volume with the same therapeutic posture and the small intestinal irradiation volume with the same irradiation technique. Results there was no significant difference in body position target area PTV and small intestine irradiation volume between the two groups (P0.05), and the volume of bladder irradiation in prone position was significantly smaller than that in supine position (P0.05). The radiation volume of V 45 was significantly higher than that of IMRT, in prone position 3D-CRT radiotherapy (P0.05). The irradiation volume of small intestine V45 in supine 3D-CRT was significantly higher than that in IMRT, (P0.05). In supine position, V45 was significantly higher in supine position than in prone position (P0.05), while in supine position V45 was significantly higher than that in prone position (P0.05). The difference was statistically significant (P0.05). The small intestine V45 in supine position was significantly higher than that in prone position under). IMRT radiotherapy (P0.05). Conclusion under the same posture, the dose of IMRT is lower than that of 3DCRT, and the V45 of prone position is smaller than that of 3DCRT, which suggests that the irradiation in prone position can reduce the dose of small intestine irradiation.
