【摘要】：目的:為干預(yù)措施制訂提供數(shù)據(jù)支持,促進(jìn)臨床合理用藥。方法:選擇2013年9-12月福建省立醫(yī)院心血管外科出院的手術(shù)患者,設(shè)計(jì)心血管外科手術(shù)患者預(yù)防使用質(zhì)子泵抑制劑(PPIs)點(diǎn)評(píng)表,收集患者基本信息、住院天數(shù)、臨床診斷、手術(shù)信息及PPIs使用情況,參照《應(yīng)激性潰瘍防治建議》及藥品說(shuō)明書(shū)對(duì)PPIs使用情況進(jìn)行專項(xiàng)點(diǎn)評(píng)。結(jié)果:總共點(diǎn)評(píng)229份病歷,其中187份使用了PPIs,使用率為81.7%,泮托拉唑的使用頻次最高;專項(xiàng)點(diǎn)評(píng)結(jié)果顯示,療程過(guò)長(zhǎng)是最常見(jiàn)的不合理用藥表現(xiàn),另外還存在用藥頻次錯(cuò)誤、無(wú)指征更換藥物、明顯藥物相互作用、重復(fù)用藥、無(wú)適應(yīng)證用藥及溶劑選擇錯(cuò)誤等不合理現(xiàn)象。結(jié)論:此次專項(xiàng)點(diǎn)評(píng)發(fā)現(xiàn)PPIs在臨床使用過(guò)程中存在諸多不足,將根據(jù)點(diǎn)評(píng)結(jié)果制訂行政及技術(shù)干預(yù)措施,并持續(xù)改進(jìn),促進(jìn)PPIs的合理使用。
[Abstract]:Objective: To provide data support for intervention measures and to promote the rational use of drugs. Methods: From September to December,2013, the patients who were discharged from the cardiovascular surgery in Fujian province were selected, and the patients with cardiovascular surgery were designed to prevent the use of the proton pump inhibitor (PPIs). The basic information of the patients, the number of days of hospitalization, the clinical diagnosis, the operation information and the application of PPIs were collected. Please review the application of PPIs with reference to the recommendations for prevention and control of stress and the use of PPIs. Results: A total of 229 medical records were reviewed,187 of which used PPIs, the rate of utilization was 81.7%, and the frequency of the use of torasola was the highest; the results of the special review indicated that the course of treatment was the most common irrational drug performance, and there was also an error in the frequency of the medication and no indication of the replacement of the drug. Obvious drug interaction, repeated use, non-adaptive drug and solvent selection error and other unreasonable phenomena. Conclusion: The special review found that PPIs have a lot of defects in clinical use, and will develop administrative and technical intervention measures according to the results of the review, and continuously improve and promote the rational use of PPIs.
