【摘要】：臨床起效緩慢是目前抗抑郁藥物治療的明顯不足,因此迫切需要快速起效抗抑郁藥物�；谝褕�(bào)道抗抑郁藥物作用特點(diǎn),根據(jù)起效時(shí)間分為快、中、慢速三類:快速特指數(shù)小時(shí)內(nèi)起效,如NMDA受體拮抗劑;中速特指7 d內(nèi)起效,包括乙酰膽堿M受體拮抗劑、5-HT_(2c)受體拮抗劑、5-HT_4受體激動(dòng)劑、5-HT_7受體拮抗劑、ATP和乙酰左旋肉毒堿等;慢速特指14 d以上起效,包括傳統(tǒng)單胺轉(zhuǎn)運(yùn)體抑制劑和單胺氧化酶抑制劑。根據(jù)抗抑郁藥物作用機(jī)制,提出了抗抑郁行為改變的啟動(dòng)扳機(jī)假說(shuō)解釋藥物起效快慢現(xiàn)象�？焖倨鹦Э挂钟羲幬镅芯客卣沽藛伟奉惿窠�(jīng)遞質(zhì)系統(tǒng)為靶點(diǎn)的傳統(tǒng)框架,為研發(fā)抗抑郁新藥和揭示抑郁癥病理生理機(jī)制提供了重要參考。
[Abstract]:The slow clinical effect is the obvious shortage of antidepressant treatment at present, so there is an urgent need for rapid antidepressant treatment. According to the characteristics of reported antidepressants, they can be divided into three categories according to the onset time: fast, medium and slow: rapid and specific within hours, such as NMDA receptor antagonists; 5-HT_4 receptor antagonists, 5-HT_4 receptor agonists, 5-HT_7 receptor antagonists, ATP and acetylL-carnitine were effective within 7 days, including acetylcholine M receptor antagonists, 5 / T _ (2c) receptor antagonists, HT2c receptor agonists, HT2c receptor antagonists, acetylL-carnitine and so on. Slow speed refers to the onset of more than 14 days, including traditional monoamine transporter inhibitors and monoamine oxidase inhibitors. According to the mechanism of antidepressant action, the trigger hypothesis of antidepressant behavior change was put forward to explain the rapid and slow effect of antidepressant. The study of rapid antidepressants has expanded the traditional framework of monoamine neurotransmitter system, which provides an important reference for the development of new antidepressant drugs and the disclosure of pathophysiological mechanism of depression.
