【摘要】：藥品支出上漲是全球問題。為應(yīng)對這一壓力,多數(shù)高收入國家制定并實施了一系列的藥品定價和采購政策。然而,盡管中低收入國家希望能夠有效控制藥品支出預(yù)算,但其藥品市場不規(guī)范,并缺乏可行的藥品定價或采購政策。高收入國家藥品支出通常由國家或社會醫(yī)療保險機構(gòu)支付,而中低收入國家大多是個人自付,這會給政策執(zhí)行造成阻力。由于對藥品定價和采購政策是否高效缺乏嚴謹?shù)恼{(diào)查研究,因而一定程度上阻礙了中低收入國家政策方案的實施。本文對已發(fā)表的有關(guān)藥品定價和采購政策的文章進行綜述發(fā)現(xiàn),許多有效的政策也伴隨著各種風險。全球尚沒有一種最佳的政策選擇,各國應(yīng)根據(jù)具體國情,多種政策結(jié)合使用。中低收入國家的實證研究尚不足,由于缺乏完善的法律體系以及未建立專門的藥品采購機構(gòu),任何一種政策選擇所伴隨的風險在這些國家都會引起更大的爭議,然而這或許是幫助其改善藥品定價和采購體系的關(guān)鍵因素。
[Abstract]:Rising drug spending is a global problem. In response to this pressure, most high-income countries have developed and implemented a range of drug pricing and procurement policies. However, despite the desire of low- and middle-income countries to effectively control their drug spending budgets, their drug markets are irregular and lack viable drug pricing or procurement policies. High-income countries usually pay for drugs by state or social health insurance agencies, while low- and middle-income countries mostly pay for themselves, creating obstacles to policy implementation. Due to the lack of rigorous investigation and research on the efficiency of drug pricing and procurement policies, the implementation of policy programs in low- and middle-income countries is hindered to some extent. This paper reviews the published articles on drug pricing and procurement policy and finds that many effective policies are accompanied by various risks. There is no best policy choice in the world. Countries should combine various policies according to their specific conditions. Empirical research in low- and middle-income countries is inadequate, and the risks associated with any policy choice are more controversial in those countries because of the lack of a sound legal system and the absence of specialized drug procurement agencies. But this may be a key factor in helping it improve its drug pricing and procurement systems.
